研究目的：

本研究旨在评估社区家庭医生的干预对于提高2型糖尿病患者的自我管理能力和药物依从性的影响效果及作用，从而为促进糖尿病患者自我管理能力的提升以及完善基层社区糖尿病防治管理的工作提供参考依据。  

Objectives of Study：

The study aims to assess the impact and effectiveness of interventions by community family physicians in improving self-management abilities and medication adherence among patients with type 2 diabetes. The findings will serve as a reference for enhancing the self-management capabilities of diabetes patients and improving the management of diabetes prevention and control at the grassroots community level.

纳入标准：

Inclusion criteria

排除标准：

（1）糖尿病合并有严重的并发症，如严重糖尿病肾病、糖尿病性视网膜严重病变等；（2）伴随其他严重的身体疾病；（3）有精神异常和认知障碍的患者；（4）同时患有肿瘤并且半年内接受过化疗、放疗等治疗的患者；（5）患有特殊类型糖尿病或其他不适合本研究的疾病；（6）正在参加其他研究课题的患者。

Exclusion criteria：

(1) Diabetes with serious complications, such as severe diabetic nephropathy, severe diabetic retinopathy, etc.; (2) with other serious physical diseases; (3) patients with mental disorders and cognitive impairments; (4) patients concurrently diagnosed with cancer and having received chemotherapy, radiotherapy or other treatments within the past six months; (5) suffering from special types of diabetes or other diseases not suitable for this study; (6) patients who are participating in other research projects.

试验所选取的两个区的所有社区医院都被编号。使用计算机生成的随机数，随机选择社区医院。使用西安市社区健康档案管理系统，导出所选社区医院老年人的健康信息，从1到n排序。通过随机数软件生成180个号码的相应老年人作为样本。

Randomization Procedure (please state who generates the random number sequence and by what method)：

All community hospitals in the two districts selected for the trial were numbered. Then use computer-generated random numbers to randomly select community hospitals. Use the Xi'an community health file management system to export the health information of the elderly in the selected community hospital, sorted from 1 to n. Generate corresponding random numbers for 180 individuals using random number software to serve as the sample.
