研究目的：

1.本项目以卒中后运动性失语为研究对象，基于认知心理学和语言学理论，阐明执行功能影响语义加工及言语产出过程的认知心理机制；2.在常规语言康复的基础上，增加关注执行功能的康复方案，利用神经影像及信息处理技术，探讨执行功能康复促进语言加工过程的动态脑网络可塑性机制。

Objectives of Study：

1.This project takes post-stroke Broca's aphasia (BA) as the research object. Based on cognitive psychology and linguistic theories, the cognitive psychological mechanism of executive function affecting semantic processing and speech production is clarified; 2.On the basis of conventional speech rehabilitation, this project adds rehabilitation programs focusing on executive function, and uses neuroimaging and information processing technology to explore the plasticity mechanism of dynamic brain network in the promotion of speech processing by executive function rehabilitation.

纳入标准：

Inclusion criteria

排除标准：

1.发病前已有语言功能障碍，包括但不限于失语、构音障碍、言语失用； 2.发病后存在失语症以外的语言障碍类型，包括但不限于构音障碍、言语失用； 3.发病前已有认知障碍病史，包括淀粉样脑血管病、伴有皮质下梗死和白质脑病的常染色体显性遗传性脑动脉病、神经系统变性病等可能引起认知障碍的疾病史； 4.神经影像学发现脑白质病变、微出血、血管周围间隙扩大、脑萎缩、腔隙性梗死等脑小血管病变； 5.存在无法完成评估的情况：意识障碍，格拉斯哥昏迷评分＜15分；右侧肢体肌力≤3级无法配合完成评估； 6.存在潜在危险的既往病史：癫痫病史；精神健康问题病史；严重的视力、听力障碍；严重的心、肺、肝、肾功能不全； 7.入组前接受任何类型的语言及认知治疗； 8.存在MRI检查禁忌症（幽闭恐惧症、体内不可摘卸金属支架、人工耳蜗等）； 9.未签署知情同意书者。

Exclusion criteria：

1.Pre-onset language dysfunction, including but not limited to aphasia, dysarthria, and speech apraxia; 2.There are types of speech disorders other than aphasia after the onset, including but not limited to dysarthria and speech apraxia; 3.Prior to onset, there was a history of cognitive impairment, including amyloid cerebrovascular disease, autosomal dominant cerebral artery disease accompanied by subcortical infarction and leukoencephalopathy, neurodegenerative disease and other diseases that may cause cognitive impairment; 4.Neuroimaging findings showed small cerebral vascular lesions such as white matter lesions, microbleeding, perivascular space enlargement, cerebral atrophy, lacunar infarction, etc. 5.Failure to complete the assessment: disturbance of consciousness, Glasgow coma score<15; the muscle strength of the right limb≤grade 3 that cannot be matched to complete the evaluation; 6.Potentially dangerous past medical history: history of epilepsy; history of mental health problems; severe visual and hearing impairment; severe heart, lung, liver and kidney insufficiency; 7.Received any type of speech and cognitive therapy before enrollment; 8.There are contraindications for MRI examination (claustrophobia, inremovable metal stent, cochlear implant, etc.); 9.Informed consent has not been signed.

盲法：

患者及家属对分组情况是不明的。执行言语-语言疗法（speech and language therapy，SLT）或数据收集与处理的参与者和医生对分组情况也是不明的。由于重复经颅磁刺激（repetitive transcranial magnetic stimulation，rTMS）干预的性质，进行rTMS干预的医生不能被盲法，因此他们不参与研究过程。设计师和负责设置盲法的工作人员也不参与整个干预过程。

Blinding：

Patients and their families are unclear about the grouping situation. During the SLT, the participants and physicians are blind to the groups. Those collecting and processing the data are also blind to the groups. Due to the nature of the rTMS intervention, physicians cannot blind themselves. They are thus not involved in the research process. The designer and the staff responsible for allocation concealment will not take part in the entire intervention.
