研究目的：

研究和评估音乐干预对妇科腹腔镜手术患者综合术后疼痛和长期焦虑情绪的影响。

Objectives of Study：

To study and evaluate the effects of music intervention on comprehensive postoperative pain and long-term anxiety in gynecological laparoscopic surgery patients.

药物成份或治疗方案详述：

患者随机分配至实验组和对照组。实验组采用音乐疗法+常规治疗；对照组采用常规治疗。关于音乐疗法，音乐选择中山大学附属第三医院微信小程序【云上三院】中的“音乐调节情绪模块”中所有音乐类型，术前 1 天开始，以患者舒适音量为主，每天听30 分钟，至术后 30 天为 1 个疗程。常规治疗包括基础治疗+腹腔镜下全子宫切除术。关于基础治疗，包括饮食控制、血糖控制、血压控制等。关于腹腔镜下全子宫切除术，（1）术前准备及麻醉：术前天阴道灌洗上药，术前小时禁食，清洁灌肠，并行备皮，清洁脐孔等术前准备。麻醉采用气管插管，静脉麻醉与吸入麻醉复合全身麻醉。（2）手术器械及使用方法：1）气腹系统：二氧化碳、气腹机、气腹针、进气管等；2）冷光源系统；3）摄像显示系统：奥林巴斯摄像头、奥林巴斯腹腔镜、信号转换器、监视器（显示屏）等；4）单极或者双极高频电刀及奥林巴斯超声刀；5）腹腔镜器械：套管针、电凝勾、抓钳、分离钳等；（3）操作步骤：在患者脐上缘，双侧髂前上棘内上 3cm 及左侧切口外上约一拳位置作四个切口，置入 Trocar。助手医师从阴道放入举宫杯用于举宫。双极电刀和超声刀依次切断双侧圆韧带、输卵管峡部及卵巢固有初带，剪开子宫膀胱腹膜返折，分离钳下推膀胱，分离宫旁组织，暴露子宫血管并结扎，双极电刀和超声刀切断双侧子宫骶骨韧带，切断双侧主韧带至宫颈外口水平，暴露阴道穹窿，单极电钩或超声刀环形切开整个阴道穹窿部，经阴道取出子宫。消毒阴道，消毒敷料卷套上橡胶手套后放入阴道，重新充气后可吸收线连续缝合阴道残端盆腔腹。冲洗盆腹腔，检査无明显出血后，尽量排空腹腔内二氧化碳气体，缝合各穿刺孔。

Description for medicine or protocol of treatment in detail：

Patients were randomly assigned to the experimental and control groups. The experimental group received music therapy + basic therapy + laparoscopic total hysterectomy. The control group received basic treatment plus laparoscopic total hysterectomy. For music therapy, all types of music in the "Music regulating emotion module" in the wechat mini program of the Third Affiliated Hospital of Sun Yat-sen University were selected. Starting from 1 day before surgery, patients' comfortable volume was the main object, and 30 minutes a day was listened to until 30 days after surgery. About basic treatment, including diet control, blood sugar control, blood pressure control, etc. About laparoscopic total hysterectomy, (1) preoperative preparation and anesthesia: the day before the operation of vaginal irrigation, fasting hours before the operation, clean enema, concurrent skin preparation, cleaning of the umbilical hole and other preoperative preparation. Anesthesia was performed by tracheal intubation, intravenous anesthesia and inhalation anesthesia combined with general anesthesia. (2) Surgical instruments and methods of use: 1) pneumoperitoneum system: carbon dioxide, pneumoperitoneum machine, pneumoperitoneum needle, intake pipe, etc. 2) cold light source system; 3) camera display system: Olympus camera, Olympus laparoscope, signal converter, monitor (display), etc.; 4) unipolar or bipolar high-frequency electric knife and Olympus ultrasonic knife; 5) laparoscopic instruments: trocar, electric coagulation hook, grasping forceps, separation forceps, etc.; (3) Procedure: Four incisions were made at the superior umbilical margin of the patient, 3cm above the internal anterior superior iliac spine and about one fist above the external incision on the left side, and Trocar was implanted. The assistant physician puts the cup into the vagina to lift the uterus. The bipolar electric knife and ultrasonic knife cut the bilateral round ligament, the isthmus of the fallopian tube and the initial band of the ovary in turn, cut the uterine vesicoperitoneal fold, separate the bladder with forceps, separate the paruterine tissue, expose the uterine blood vessels and ligation, the bipolar electric knife and ultrasonic knife cut the bilateral uterosacral ligament, cut the bilateral principal ligament to the level of the external cervix, and expose the vaginal fornix. Unipolar electric hook or ultrasonic knife cut the entire vaginal fornix and remove the uterus through the vagina. To disinfect the vagina, put the disinfected dressing roll on the rubber gloves and put it into the vagina. After re-aerating, the absorbent thread can be used to suture the pelvic stump of the vagina continuously. After washing the abdominal cavity and checking no obvious bleeding, the abdominal cavity was emptied of carbon dioxide gas as far as possible, and each puncture hole was sutured.

1.恶性肿瘤患者； 2.有神经和/或认知问题的人，不能理解研究性质、范围或不能遵守医嘱者； 3.不喜欢音乐的人； 4.患有慢性疼痛的人； 5.听力障碍的人； 6.不同意入组患者。

Exclusion criteria：

1. Patients with malignant tumors; 2. People with neurological and/or cognitive problems, inability to understand the nature or scope of the study, or inability to follow medical orders; 3. Patients who dislike music; 4. Patients with chronic pain; 5. Patients with hearing impairment; 6. Patients who do not agree to enroll.

本试验采用随机分配方法。由统计人员采用SPSS 25.0随机数字表，产生随机号，按符合纳入排除标准的病例进入研究的先后顺序，研究者根据随机号对应的方案安排患者入组研究。
