研究目的:

经皮肾镜取石术(PCNL)是治疗上尿路结石的主要微创手术方式,其关键基础是准确地穿刺目标肾盏。B超引导是PCNL穿刺引导的主流方式,但B超引导对穿刺针造影不够清晰且只能观察其部分针体,并且B超引导只能提供二维信息,容易导致位置误判,造成出血、集合系统穿孔等风险。我们研发一项基于光学定位的穿刺导航技术,基于光学定位的穿刺引导系统针对采用超声引导的穿刺手术,提供穿刺针的位置、轨迹、姿态等可视化信息,并与现有超声图像实时叠加显示,术前到术中全程辅助医生掌握穿刺路径,有效提升穿刺准确度与效率,缩短学习曲线。 本项目首次利用多轴力反馈方案,基于压力传感器和单悬臂梁近似,建立穿刺针以及穿刺轨迹的模型,结合双目视觉进行三维空间定位,从而降低穿刺针弯曲导致的穿刺误差。该系统为穿刺导航提供一种定位误差小且实时成像的新方式,清晰实时的导航成像技术可以提高术中穿刺精度,提高穿刺手术的成功率,减少穿刺过程对患者造成的创伤,基于前期动物实验、一二期临床研究成果,计划进行单中心、前瞻、单盲、配对临床研究以证明该技术的安全性与有效性。  

Objectives of Study:

Percutaneous nephrolithotomy (PCNL) is a major minimally invasive surgical procedure for the treatment of upper urinary tract stones, and its key foundation is the accurate puncture of the target renal calyx.Ultrasound guidance is the mainstream method of puncture guidance for PCNL, but ultrasound guidance is not sufficiently clear to visualize the puncture needle and can only observe part of the needle, and ultrasound guidance can only provide two-dimensional information, which is prone to positional miscalculation, resulting in the risk of bleeding, perforation of the collecting system, and so on. We have developed an optical localization-based perforation needle imaging system. We have developed a puncture navigation technology based on optical localization. The optical localization-based puncture guidance system for ultrasound-guided puncture surgeries provides visual information on the position, trajectory, and posture of the puncture needle and displays it in real-time superimposed on the existing ultrasound images, assisting the surgeon in mastering the path of the puncture from pre-operative to intra-operative, effectively improving the accuracy and efficiency of the puncture and shortening the learning curve. This project is the first to utilize a multi-axis force feedback scheme, based on pressure sensors and a single cantilever beam approximation, to establish a model of the puncture needle as well as the puncture trajectory, combined with binocular vision for 3D spatial localization, so as to reduce the puncture error caused by the bending of the puncture needle. This system provides a new way for puncture navigation with small positioning error and real-time imaging, and the clear real-time navigation imaging technology can improve the intraoperative puncture accuracy, increase the success rate of puncture surgery, and reduce the trauma caused to patients during the puncture process. Based on the results of the preliminary animal experiments, phase I and II clinical research, a single center, prospective, single-blind, paired clinical study is planned to prove the safety and efficacy of this technology.

排除标准:

(1)排除患有马蹄形、盆腔肾或尿脓毒症的患者; (2)手术有禁忌症者; (3)上尿路上皮癌史、凝血功能障碍病史; (4)存在任何导致无法配合治疗的情况(如精神类疾病)者。

Exclusion criteria:

(1) Exclude patients with equinus, pelvic kidney, or urinary sepsis; (2) Patients with contraindications to surgery; (3) History of epithelial cancer of the upper urinary tract and history of coagulation disorders; (4) Presence of any condition (e.g., psychiatric disorders) that makes it impossible to cooperate with the treatment.
