研究目的：

探究甲苯磺酸瑞马唑仑与右美托咪啶对关节置换手术老年患者术后谵妄的影响，以期寻求合适的药物，降低老年患者术后谵妄的发生，从而改善患者预后。  

Objectives of Study：

To explore the effects of remimazolam tosilate and dexmedetomidine on postoperative delirium in elderly patients undergoing joint replacement surgery, in order to find suitable drugs to reduce the occurrence of postoperative delirium in elderly patients and improve the prognosis.

纳入标准：

Inclusion criteria

排除标准：

1.拒绝参加本研究； 2.存在椎管内麻醉禁忌症； 3.严重心、脑、肺、肝、肾等主要脏器疾病的患者； 4.对苯二氮卓类药物或者右美托咪啶过敏患者； 5.既往有谵妄、痴呆等精神病史。

Exclusion criteria：

1. Refuse to participate in the study; 2. Contraindications to spinal anesthesia; 3. History of serious major organ diseases, like heart, brain, lung, liver, kidney; 4. Allergic to benzodiazepines or dexmedetomidine; 5. History of delirium, dementia and other mental illnesses.

项目负责人使用SAS软件生成随机数字，采用区组随机分组，区组大小为6，将患者随机分成对照组、瑞马唑仑组和右美托咪定组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The project leader used SAS software to generate random numbers. Each patients was randomly divided into control group, remimazolam group, or dexmedetomidine group in a 1:1:1 ratio with a random block size of 6.

盲法：

本研究对患者、手术医生实施盲法；术后随访和数据录入人员不参与随机化和围术期管理；由于两种药物注药速度不同，本研究对麻醉医生不设盲。

Blinding：

In this study, patients and surgeons were blinded. Postoperative follow-up and data entry personnel were not involved in randomization and perioperative management; because the two drugs were administered at different speeds, anesthesiologists were not blinded in this study.
