研究目的：

以探讨真实世界中TNF-α抑制剂对老年RA患者脑白质疏松、认知功能及脑血流动力学的影响，从生物制剂类药物抑制类风湿患者炎症反应的角度观察其对老年RA患者脑白质疏松、认知功能及脑血流改变影响，从脑白质解剖学、脑血流动力学等方面探讨其可能的作用机制。  

Objectives of Study：

To explore TNF in the real world- α The effects of inhibitors on white matter osteoporosis, cognitive function, and cerebral hemodynamics in elderly RA patients. From the perspective of biological agents inhibiting inflammatory response in rheumatoid arthritis patients, the effects of inhibitors on white matter osteoporosis, cognitive function, and changes in cerebral blood flow in elderly RA patients were observed. The possible mechanisms of action were explored from aspects such as white matter anatomy and cerebral hemodynamics.

纳入标准：

Inclusion criteria

排除标准：

1.血糖、血压控制不佳或存在肝肾功能不全、血液系统、消化道溃疡疾病、肿瘤、急慢性感染等； 2.伴有其他风湿免疫类疾病或存在急性中枢炎性脱髓鞘疾病； 3.对甲氨蝶呤或益赛普不能耐受或出现明显不良反应； 4.不能配合复诊。

Exclusion criteria：

1. Poor control of blood sugar and blood pressure, or presence of liver and kidney dysfunction, hematological system, gastrointestinal ulcer diseases, tumors, acute and chronic infections, etc; 2. Accompanying other rheumatic and immune diseases or having acute central inflammatory demyelinating diseases; 3. Inability to tolerate or exhibit significant adverse reactions to Methotrexate or Yisaipu; 4. Unable to cooperate with follow-up visits.
