研究目的：

探究允许性高碳酸血症在单肺通气胸科手术中对脑氧饱和度及术后认知功能障碍的作用研究。  

Objectives of Study：

To explore the effect of permissive hypercapnia on cerebral oxygen saturation and postoperative cognitive dysfunction in thoracic surgery with one-lung ventilation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1.急诊手术患者； 2.术前中枢神经系统疾病史：既往颅内高压、颅脑手术史、脑血管疾病史（TIA、脑梗死、脑出血）、颅脑外伤史等； 3.术前合并严重肺功能障碍：术前肺炎、肺不张、胸腔积液、急性肺损伤、ARDS； 4.术前合并严重肝肾功能不全：Child-Pugh 分级 B、C 级患者、需规律行 CRRT 的患者； 5.术前合并严重心血管疾病：急性冠状动脉综合征、LVEF＜45%、二度、三度房室传导阻滞； 6.静息状态下吸空气 SpO2 不能维持 90%以上； 7.术前认知功能受损：术前 mini-cog 评分＜2 分或 MMSE 评分低（文盲＜17 分、小学文化水平＜20 分、初中及以上文化水平＞24 分）； 8.术中经麻醉医生评估无需建立连续有创动脉压监测及开放中心静脉通路的患者。

Exclusion criteria：

1. Emergency surgery patients; 2. History of central nervous system diseases: intracranial hypertension, craniocerebral surgery, cerebrovascular disease (TIA, cerebral infarction, cerebral hemorrhage), craniocerebral trauma; 3. Severe pulmonary dysfunction: preoperative pneumonia, atelectasis, pleural effusion, acute lung injury, ARDS; 4. Severe liver and kidney dysfunction: Child-Pugh grade B, C, patients who need regular CRRT; 5. Severe cardiovascular disease: acute coronary syndrome, LVEF < 45 %, second-degree, third-degree atrioventricular transmission Conduction block; 6. SpO2 can not maintain more than 90% in the resting state; 7. Preoperative cognitive dysfunction: Preoperative Mini-Cog scale score < 2 points; 8. Patients who need radial artery catheterization and central venous catheterization during operation.

对患者行脑氧监测后，由一名不参与研究的第三方对病人的脑氧情况做连续记录，并将监护显示器摆放在施盲麻醉医生无法看到的位置，施盲麻醉医生对病人进行常规麻醉管理而不根据脑氧饱和度指标的变化进行相关干预。双盲。

Blinding：

After the patient underwent cerebral oxygen monitoring, the patient 's cerebral oxygen was continuously recorded by a third party not involved in the study, and the cerebral oxygenation monitoring display was placed in a position that was not visible to the blind anesthesiologist. The blind anesthesiologist performed routine anesthesia management on the patients, and did not intervene according to the changes of cerebral oxygen saturation index. Double blinded.
