本研究以Halo Sport2设备(智能耳机,可以通过内置在耳机头带的电极,对受试者进行经颅直流电刺激)进行的经颅直流电刺激,针对慢性非特异性腰痛(NS-CLBP下同)患者人群,入组62例,分为真刺激组(n=31例)和假刺激组(n=31例),所入组患者均联合运动治疗。研究拟探索以初级运动皮层为干预靶点,利用经颅直流电刺激技术联合运动训练,探讨经颅直流电刺激联合运动训练能否改善患者腰痛、腰部功能、认知功能,大脑的结构、功能连接,并进一步讨论可能的治疗机制。基于初级运动皮层电刺激镇痛通路的理论,提出Halo Sport2能加强常规治疗NS-CLBP效果的假设。 |

Objectives of Study: |
In this study, a total of 62 patients with chronic non-specific low back pain (NS-CLBP) were enrolled with transcranial direct current stimulation performed by Halo Sport2 device (smart headset, which can provide transcranial direct current stimulation to subjects through electrodes embedded in the headband of the headset). They were divided into true stimulation group (n=31 cases) and false stimulation group (n=31 cases). All patients were treated with exercise. This study intends to explore the primary motor cortex as the intervention target, and use transcranial direct current stimulation technology combined with exercise training to explore whether transcranial direct current stimulation combined with exercise training can improve patients' low back pain, lumbar function, cognitive function, brain structure and functional connectivity, and further discuss possible therapeutic mechanisms. Based on the theory of electrical stimulation of the primary motor cortex analgesic pathway, the hypothesis that Halo Sport2 can enhance the effect of conventional treatment of NS-CLBP was proposed. |

排除标准: |
(1)疼痛部位为腰背部,但由于类风湿关节炎、强直性脊柱炎、脊柱肿瘤、骨折、感染、椎间盘突出或马尾综合征等明确的病理改变。以脊髓压迫或明显神经根损伤为症状的腰痛,伴严重的双下肢感觉运动异常 (2)重度高血压、糖尿病患者 (3)精神疾病 (4)使用心脏起搏器或其他植入装置 (5)怀孕 (6)吸毒 (7)滥用药物和酒精饮料 (8)抑郁量表(SDS),得分>50 (9) 6个月前有腰椎手术史的患者 (10)既往tDCS治疗 (11)治疗期间参加其他试验的 (12)有其他慢性疼痛综合征的患者 (13)认知障碍、文盲或沟通障碍 (14)有幽闭恐惧症、金属植入物或其他与MRI检查相关的禁忌症的患者。 |

Exclusion criteria: |
(1) The pain site is low back, but due to rheumatoid arthritis, ankylosing spondylitis, spinal tumor, fracture, infection, disc herniation, or cauda equina syndrome and other definite pathological changes. Low back pain with symptoms of spinal cord compression or obvious nerve root injury associated with severe sensorimotor abnormalities in both lower limbs (2) Severe hypertension and diabetes mellitus (3)Mental illness (4)Use of a pacemaker or other implanted device (5) Pregnancy (6)Drug use (7) Misuse of drugs and alcoholic beverages (8)Depression Scale (SDS), score > 50 (9)Patients with a history of previous lumbar surgery 6 months ago (10)previous tDCS treatment (11)those who participated in other trials during the treatment period (12)patients with other chronic pain syndromes (13) cognitive impairment, illiteracy or communication impairment (14)patients with claustrophobia, metal implants or other contraindications related to MRI examination. |
