研究目的：

探讨地塞米松复合右美托咪定辅助用于肌间沟臂丛神经阻滞的镇痛效果，优化上肢手术的术后镇痛管理策略，为临床应用推广提供依据。

Objectives of Study：

To investigate the analgesic effect of dexamethason combined with dexmedetomidine for interscalene brachial plexus block, so as to optimize postoperative analgesia management strategy for upper extremity surgery.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

Inclusion criteria

排除标准：

1.妊娠、哺乳期妇女； 2.病态肥胖（BMI≥40kg/m2)； 3.严重心、脑、肝、肾疾病患者； 4.上肢瘫痪或肢体感觉、运动障碍患者； 5.有精神疾病、不能正常沟通或不能正确进行VAS评分等患者； 6.对酰胺类局麻药、地塞米松和右美托咪定等药物过敏患者； 7.有阿片类药物或毒物滥用史患者。

Exclusion criteria：

1. Pregnant and lactating patients; 2. Morbid obesity (BMI>=40kg/m^2); 3. Patients with severe heart, brain, liver and kidney diseases; 4. Patients with upper limb paralysis or limb sensory and movement disorders; 5. Patients with mental illness, inability to communicate properly, or inability to correctly score VAS; 6. Patients allergic to amides local anesthetics, dexamethasone and dexmedetomidine; 7. Patients with a history of opioid or drug abuse.

随机数字预先由PASS 15软件生成，并分为四组：对照组（C组）、地塞米松组（D组）、右美托咪定组（Y组）及地塞米松复合右美托咪定组（DY组）。

The random numbers were generated by PASS 15 software and divided into four groups: control group (Group C) , dexamethasone group (Group D) , dexmedetomidine group (Group Y) and dexamethasone combined with dexmedetomidine group (Group DY).

盲法：	本试验采用双盲，对受试者、术后观察人员和统计分析者均设盲。A医生把分配序列打印后放在信封里密封，一式两份，原文件由A医生保管，并设置密码，分配序列锁在抽屉里，别人无法获得，另一份交给研究主要负责人。本研究术后观察人员不参与随机分组，也不参与术中研究实施。
Blinding：	The double-blind method was used in this study. The subjects, postoperative observers and statistical analyzers were blinded. Dr. A printed the distribution sequence and put it in a sealed envelope in duplicate. Dr. A kept the original file and set the password. The distribution sequence was locked in a drawer so that no one else could get it, another was given to the lead researcher. Postoperative observers did not participate in randomization, nor did they participate in the implementation of the intraoperative study.
