研究目的：

1 了解慢性心衰患者在心衰健康知识及移动健康教育程序的需求。 2 开发一套慢性心衰患者健康教育的微信小程序。 3 评价该微信小程序在慢性心衰患者自我护理能力、生活质量、心衰知识水平方面的干预效果。  

Objectives of Study：

1. Understand the needs of chronic heart failure patients in terms of heart failure health knowledge and mobile health education programs. 2. Develop a WeChat mini program for health education for patients with chronic heart failure. 3. Evaluate the intervention effect of this WeChat mini program on self-care ability, quality of life, and knowledge level of chronic heart failure patients.

Inclusion criteria

排除标准：

1.存在意识或认知障碍; 2.合并其他重大及慢性疾病； 3.处于慢性心力衰竭急性发作期者

Exclusion criteria：

1. Existence of consciousness or cognitive impairment; 2. Merge other major and chronic diseases; 3. Individuals in the acute phase of chronic heart failure
