研究目的:

1.开展随机对照试验研究,证实运动结合认知训练对脑卒中患者的干预效果,为其临床转化提供循证医学证据支撑; 2.采集训练前后脑卒中患者的表面肌电信号进行协同肌肉分析,探究脑卒中患者下肢步行时肌肉的激活模式及活动效率,从运动生物力学角度分析运动认知结合训练对改善脑卒中患者在反应性姿势控制时的动作协调性及姿势控制策略的有效性; 3.通过功能性近红外光谱成像技术(Functional near-infrared spectroscopy,fNIRS)探究脑卒中患者在执行运动认知结合任务下和训练后的脑区激活及功能网络连接的特异性,用以指导临床康复策略的制定和完善相关神经调控机制研究。  

Objectives of Study:

1. To conduct randomized controlled trials to confirm the intervention effect of combining exercise with cognitive training on stroke patients, and provide evidence-based medical evidence support for their clinical transformation; 2. To collect surface electromyographic signals of stroke patients before and after training for collaborative muscle analysis, explore the activation mode and activity efficiency of lower limb muscles during walking in stroke patients, and analyze the effectiveness of combining motor cognition with training in improving action coordination and posture control strategies in stroke patients from the perspective of motor biomechanics; 3. To explore the specificity of brain area activation and functional network connectivity in stroke patients during motor cognitive integration tasks and after training using Functional Near Infrared Spectroscopy (fNIRS), in order to guide the development of clinical rehabilitation strategies and improve research on related neural regulatory mechanisms.

纳入标准:

Inclusion criteria

排除标准:

1.脑卒中发病前即存在平衡功能障碍; 2.心、肝、肾、脑功能衰竭等严重合并症的患者; 3.有认知障碍、精神障碍的患者; 4.不能配合完成训练者(包括失明、失聪、重度语言障碍)。

Exclusion criteria:

1. There is balance dysfunction before the onset of stroke; 2. Patients with severe comorbidities such as heart, liver, kidney, and brain failure; 3. Patients with cognitive or mental disorders; 4. Individuals who are unable to cooperate in completing training (including blindness, deafness, and severe language impairment).

Intervention:

The subject stands in a square corner (50cm * 50cm), and the experimenter adjusts the computer to issue numerical instructions based on the patient's reaction time. The subject moves towards the other three corners according to the instructions; If the instruction is "1-4", the practitioner moves from corner # 1 to corner # 4. There are a total of three selectable movement directions that must be completed in one step. Each action may be forward, backward, left, right, or diagonal. During the training process, there are a total of 12 motion commands (4 corners * 3 directions=12) and one non motion command (for example, when the command is "4-4", the patient moves from corner # 4 to corner # 4, i.e. stops moving). On the basis of regular rehabilitation training once a day, 40 minutes per session, 5 times a week, for a total of 4 weeks, a combination of exercise cognition training is conducted weekly under the guidance of professional personnel, with each training lasting 10-15 minutes.

将符合纳入标准的患者按照进入试验的先后顺序依次编号,将编号录入到 SPSS26.0软件产生随机数字,再将生成的随机数字排序产生一个新变量,即随机数字秩,最后将随机数字秩用SPSS26.0软件分成试验组和对照组。将患者分为运动认知结合训练组20例、单纯运动训练组20例、常规组20例,并制作随机分组卡。将随机分组卡进行分配隐藏,将其放入密封、不透光的信封中。当接收研究对象时,对号启封入组。

Randomization Procedure (please state who generates the random number sequence and by what method):

Number patients who meet the inclusion criteria in the order of entry into the trial, input the numbers into SPSS26.0 software to generate random numbers, and then sort the generated random numbers to generate a new variable, namely the random number rank. Finally, divide the random number rank into the experimental group and the control group using SPSS26.0 software. Divide the patients into a combination of exercise cognition training group of 20 cases, a simple exercise training group of 20 cases, and a routine group of 20 cases, and create a random grouping card. Distribute and hide random grouping cards and place them in sealed, opaque envelopes. When receiving the research object, the corresponding number is unsealed and included in the group.
