研究目的:

本项目主要研究二代测序技术在感染病精准诊断中的价值、标准试验流程制定及推广。 明确病原体种类及其耐药性是感染病治疗中最为重要的环节之一,能够帮组临床医生开展合理用药,有效治疗。但由于能够感染人类的细菌、真菌、病毒种类众多,传统的检测手段如细菌培养、PCR扩增以及免疫反应等,检测通量较低,一般一次检测仅能针对少数几种病原体,为了明确诊断结果,通常需要进行多次检测,耗时较长;二代测序技术是最近发展起来的一种快速、高通量的获得核酸序列信息的技术,而临床常见病原体均含有核酸,且能够通过核酸序列明确病原体的种类及耐药情况,这使得二代测序技术有可能成为一种快速的、普适的感染病诊断方法。 自2014年在新英格兰医学杂志上首次报道一例用二代测序技术诊断钩端螺旋体感染的病例以来,其诊断价值迅速被认可。我们前期已采用二代测序技术对近两百例临床样本进行了检测,结果与临床情高度符合;二代测序技术对各个步骤的操作均有较高要求,为了使其能够更好的服务于临床,本项目拟在前期工作的基础上进一步优化检测流程,扩大样本数量,与其它医院组建多中心临床试验,全面评估二代测序技术在呼吸道感染、血流感染、中枢神经系统感染等各种感染病中的诊断价值;制定检测流程中各个环节的标准操作,如标本采集、核酸提取、建库流程以及报告解读等;并以此标准操作流程,将二代测序技术诊断感染性疾病推广到更多医院。 通过本项目的实施将对感染病的诊断方法带来重大改变,显著提升我国的感染病诊断能力,达到世界先进水平。  

Objectives of Study:

This project mainly studies the value of the next generation sequencing technology in the diagnosis of infectious diseases. Diagnosis of pathogen is one of the most important factors for clinicians to carry out rational use of drugs, effective treatment. However, the traditional detection methods such as bacterial culture, PCR amplification and immune response, detection flux has limited sensitivity. The next-generation sequencing is a recently developed high-throughput technology to obtain nucleic acid sequence information, which makes the next-generation of sequencing technology may become a fast, universal infection diagnosis method. The diagnostic value has been rapidly recognized since the first case publish in the New England Journal of Medicine, which reported a case of diagnosis of leptospirosis with next-generation sequencing technique. The project is based on the preliminary work to further optimize the detection process and expand the number of samples, in order to achieve a comprehensive assessment of next-generation sequencing technology in respiratory infections, bloodstream infections, central nervous system infections and etc. Through the implementation of this project, we hope to enhance the diagnosis of infectious diseases in China into the world advanced level.

1. 患者主要诊断为肿瘤、自身免疫性疾病等非感染性疾病;
2. 患者第一次采集标本时病情已平稳,不能满足入选标准;
3. 在满足必需临床检验后,剩余的标本量不能满足二代测序要求。

Exclusion criteria:

1. Patients diagnosed as non-infectious diseases such as cancer and autoimmune diseases;
2. The patient is stable and can not meet the selection criteria;
3. After the necessary clinical tests, the remaining specimen can not meet the requirements of the second generation sequencing.

细菌真菌以培养法作为金标准,寄生虫,病毒,不典型病原体等根据临床需要及临床条件,可采用分离培养法、核酸检测法、抗原检测法、直接涂片镜检法等加以诊断

Culture is the gold standard for bacterial and fungi diagnosis. When diagnose parasites, viruses, and atypical pathogens, pathogen isolation, culture, nucleic acid detection, antigen detection, direct smear microscopy can can be used for clinical diagnosis standard.

患者高度疑似感染性疾病,主要诊断为以下之一:发热待查、血流感染(菌血症、败血症、感染性心内膜炎)、呼吸道感染(慢性支气管炎因感染因素急性发作、肺炎)、中枢神经系统感染(脑脊膜炎、脑炎、脑膜脑炎、脑膜炎)、浆膜腔感染(腹膜炎、胸膜炎、心包炎)、局灶脓肿(脑脓肿、肺脓肿、腹腔脓肿、肝脓肿、皮肤软组织脓肿、脾脓肿等)
