研究目的：

在“互联网+医疗”背景下，研发一款适用于心脏机械瓣膜置换术（Mechanical Heart Value Replacement，MHVR）后患者的手机APP，并探讨基于该手机APP延续性护理在心脏机械瓣膜置换术后患者的应用效果。  

Objectives of Study：

To develop an application for patients after mechanical heart valve replacement in the context of "Internet+medicine". To explore the effect of the APP on the application of continuity of care for patients after MHVR.

纳入标准：

Inclusion criteria

排除标准：

（1）认知功能障碍、精神障碍的患者； （2）患有严重慢性病的患者（肾、呼吸、肝衰竭、恶性肿瘤、冠心病、糖尿病、脑出血等）； （3）严重心力衰竭（心功能分级Ⅳ级以上）； （4）不配合研究。

Exclusion criteria：

(1) Patients with cognitive dysfunction and mental disorders; (2) Patients with serious chronic diseases (kidney, respiratory, liver failure, malignant tumors, coronary heart disease, diabetes, cerebral hemorrhage, etc.); (3) Severe heart failure (heart function grade Ⅳ or above); (4) Not cooperating with research.
