研究目的：

本研究拟基于二元疾病管理理论，以社区老年慢性心衰患者和照顾者为二元整体，通过纵向研究设计，运用生态瞬时评估法完成二元整体出院后6个月的居家症状全面动态监测，明确可控可干预靶点；构建二元整体居家代际管理方案，形成由医学生主导的老年慢性心衰二元整体居家代际管理模式；并开展实证研究和卫生经济学评价。本研究将为实现持续有效的老年慢性心衰二元整体居家管理提供科学参考，也为创新我国老年人群慢病管理防控体系、推进健康老龄化中国建设提供新的战略选择。  

Objectives of Study：

This study aims to construct a sustainable and effective home management model for older patients with chronic heart failure and their family caregivers based on the Theory of Dyadic Illness Management. Firstly, we plan to take older community-dwelling patients with chronic heart failure and their caregivers as the dyadic participants. The ecological momentary assessment will be used to monitor the comprehensive dynamic dyadic symptoms six months after discharge through a longitudinal study, which aims to identify controllable targets. Secondly, we will develop the dyadic intergenerational management plan based on the targets and construct the dyadic intergenerational management model led by medical students. Thirdly, empirical study and economic evaluation will be carried out to test the effectiveness and value of the dyadic intergenerational management model. This study will provide scientific reference for achieving sustainable and effective home management for older patients with chronic heart failure and their family caregivers, and also provide new strategic options for innovating the chronic disease management and control system for older people and promoting the construction of healthy aging in China.

纳入标准：

Inclusion criteria

排除标准：

①近3 个月发生过心肌梗死、中风、慢性阻塞性肺疾病、肾脏衰竭、癌症等（合并上述疾病对心衰患者症状产生复合效应）；②出院后拟进入照护机构；③近3 个月参加过常规护理之外的其他干预试验。

Exclusion criteria：

1. Having experienced myocardial infarction, stroke, chronic obstructive pulmonary disease, renal failure, cancer, etc. in the past 3 months (combined with the above diseases, the symptoms of heart failure patients have a compound effect); 2. Intending to enter a care facility after discharge; 3. having participated in intervention trials other than routine care in the past three months.
