研究目的：

建立以优化生活方式结合个体化精准医疗的老年患者综合干预模式，评估早期干预对老年疾病控制达标率及预后的影响。最终形成运动、营养等早期干预老年常见疾病的防治核心技术，并作推广应用。

Objectives of Study：

to establish a comprehensive intervention pattern for the elder patients which combines optimizing life-styles and personalized precision medicine and to evaluate the effect of early intervention on the control and prognosis of diseases of old people.
