研究目的：

通过临床大样本多中心随机对照研究观察，为柔筋调膝的临床疗效提供证据支持。通过卫生经济学指标和患者满意度评价结果，为柔筋调膝手法治疗膝骨关节炎的临床决策提供证据支持。通过随访检查步态分析观察患者行走功能的改善，为临床疗效提供客观证据。将中医治疗膝骨关节炎的方案优化，提高上海市膝骨关节炎中医诊疗水平。  

Objectives of Study：

Through the clinical large sample of multi-center randomized controlled study observation, for the clinical effect of the technique of adjusting and kneading knee to provide evidence to support. Through the evaluation of health economics and patient satisfaction, it provides evidence for the clinical decision-making of knee osteoarthritis. The follow-up examination gait analysis was used to observe the improvement of walking function of patients, providing objective evidence for clinical efficacy. The Chinese medicine treatment of knee osteoarthritis program optimization, improve the level of Shanghai knee osteoarthritis diagnosis and treatment of Chinese medicine.

纳入标准：

Inclusion criteria

排除标准：

(1)膝部有外伤和手术史者
(2)膝部有肿瘤、结核、骨髓炎等疾患者
(3)牛皮癣、梅毒性神经病、褐黄病、代谢性骨病、急性创伤等
(4)严重肝肾功能不全、严重心血管疾病、糖尿病及精神疾病者

Exclusion criteria：

(1) a history of trauma and surgery on the knee;
(2) the knee with cancer, tuberculosis, osteomyelitis and other diseases;
(3) psoriasis, syphilitic neuropathy, brown yellow disease, metabolic bone disease, acute trauma;
(4) severe liver and kidney dysfunction, severe cardiovascular disease, diabetes and mental illness.

手法包括软组织手法和坐位调膝手法。软组织推拿手法干预同传统手法组；坐位调膝法：患者取坐位，医者与其相对而坐。医者将患肢的踝关节适当旋转，两手拇指置于髌骨下缘向上顶推，余四指环抱腘窝松动胫股关节，嘱患者缓慢站立后再缓缓坐下，如此重复三次。 此疗法每周予以3次治疗。

干预措施：

(1) 舒经通络，活血化瘀：患者先仰卧位，先以用?法施术于大腿股四头肌，重点在于髌骨上操作，并按揉鹤顶、血海、梁丘、伏兔等穴。患者俯卧位，以?法施于大腿后侧、腘窝及小腿后侧，并提拿委中、承山穴。 (2) 松解粘连，滑利关节：患者仰卧位，屈髋屈膝，术者一手扶按患侧髌骨，一手握持小腿远端，作屈膝腰法，配合膝关节屈伸、旋转等被动活动。续则以按揉与弹拨法交替作用在髌韧带、内外侧副韧带，重点在鹤顶、内外膝眼、阳陵泉、血海、梁丘等穴周围进行治疗，并提拿髌骨。最后于患者患膝周围施以檫法，以透热为度。每周2次,疗程4周。 此疗法每周予以3次治疗。

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

电子化数据管理 1.病例报告表(CRF)的填写与移交：完成的病例报告表由课题负责人和研究助理审查后，交数据管理人员，进行数据录入与管理工作。 2.数据的录入与修改：数据录入与管理由数据管理员负责。采用Excel数据库，进行数据录入与管理。为保证数据的准确性，由两个数据管理员独立进行双份录入并校对。 3.对病例报告表中存在的疑问。数据管理员将填写疑问解答表(DRO)，并通过研究助理向课题负责人发出询问，课题负责人应尽快解答并返回，数据管理员根据课题负责人的回答进行数据修改，确认与录入。 4.数据库锁定：在确认所建的数据库无误后，由课题负责人、数据管理人员、统计分析人员对数据库进行锁定。 5.本次试验采用电子化数据管理系统（DAS for eCDM）。该系统集成了电子病例报告表（eCRF）构建 、数据录入、数据锁定及导出及EDC系统应急计划，能够科学的对试验数据进行跟踪管理和实时监控。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Electronic data management: 1.Completion and Transfer of Case Report Form (CRF): Completed case report forms are reviewed by the project leader and research assistant, and submitted to the data management personnel for data entry and management. 2.Data entry and modification: data entry and management by the data administrator. Using Excel database, data entry and management. In order to ensure the accuracy of the data, by the two data administrators for independent entry and proofreading. 3.On the case report form the existence of questions. The data manager will fill out a question-answering form (DRO) and ask questions to the project leader through the research assistant. The data manager will answer and return the data as soon as possible.
