研究目的：

1.构建住院心衰患者出院后死亡风险的预测模型； 2.利用心衰中心数据验证该模型在真实世界的效能； 3.证实基于风险分层的医疗质量改善计划与常规护理相比，对心衰预后（全因死亡或心衰住院）的改善和医疗管理质量指标的影响。

Objectives of Study：

1.Construct a predictive model for post-discharge mortality risk in hospitalized heart failure patients; 2.Validate the efficacy of the model in the real world using heart failure center data; 3.Confirm the impact of a risk-stratified medical quality improvement plan compared to routine care on the prognosis of heart failure (all-cause mortality or heart failure hospitalization) and healthcare management quality indicators.

Inclusion criteria

排除标准：

1.院内死亡； 2.除心力衰竭外，研究者认为存在导致预期寿命<1年的任何疾病； 3.不能遵守研究评估时间表进行随访，或根据研究者的临床判断不适合进行试验； 4.慢性肾脏病需要长期肾脏替代治疗。

Exclusion criteria：

1. Death within the hospital; 2. Except for heart failure, researchers believe that there are any diseases that lead to a life expectancy of less than 1 year; 3. Failure to follow the research evaluation schedule for follow-up, or based on the clinical judgment of the researcher, the trial is not suitable; 4. Chronic kidney disease requires long-term renal replacement therapy.
