研究目的：

FAMILY试验的主要目的是评估由下肢力量-平衡训练和健康教育计划组成的综合干预措施与国家基本公共卫生服务相整合在减少 60 岁及以上成年人在随机分配后12个月内至少跌倒一次的比例方面的效果。次要目标包括对跌倒率（每人每年跌倒的次数）、功能活动能力、过去 12 个月中因跌倒而受伤的人数比例以及与健康相关的生活质量进行效果评估。  

Objectives of Study：

The primary objective of FAMILY trial is to assess the effectiveness of a combined intervention, consisting of lower-extremity strength-balance training and health education program in reducing the proportion of people who fall at least once in the 12 months after randomization among adults aged 60 years older and above. Secondary objectives include the effectiveness assessment on the rate of falls (the number of falls per person-years), functional mobility, the proportion of people who have fall-induced injury in the past 12 months and health-related quality of life.

纳入标准：

Inclusion criteria

排除标准：

1.在没有他人帮助或助行器的情况下无法行走； 2.患有无法进行运动的疾病，或经村医评估预期寿命少于 6 个月； 3.被诊断患有神经退行性疾病（如阿尔兹海默病、帕金森病）； 4.聋或/和哑。

Exclusion criteria：

1. Unable to walk without the assistance from another person or walking aids; 2. Having a medical condition precluding exercise or with life expectancy less than 6 months assessed by the village doctor;  3. Having a diagnosis of neurodegenerative diseases (e.g., Alzheimer’s disease, Parkinson’s disease); 4. Mute or deaf.

1. 运动锻炼：共有七个动作，包含有热身运动（两个动作）、上肢运动（一个动作）和下肢肌力及平衡训练（四个动作）。通过集体锻炼与居家锻炼方式相结合，集体练习至少每月一次，由乡村医生提供。自我指导的家庭练习则建议每周至少进行四次，并辅以预先录制的视频。 2. 健康教育：村医通过预先录制的视频进行健康教育，主要内容包括运动锻炼动作要点、跌倒预防知识、与跌倒相关的危险因素等，频率每三个月一次。

1.Exercise Program: The regimen consists of seven specific movements, including warm-up exercises (two movements), upper limb exercises (one movement), and lower limb strength and balance training (four movements). Implementation combines group sessions with home-based practice, where rural healthcare providers conduct organized group exercises at least monthly. Participants are advised to perform self-guided home practice a minimum of four times weekly, supported by pre-recorded instructional videos. 2.Health Education: Village medical practitioners deliver health education through pre-filmed video materials triannually (once every three months). Core content covers proper exercise techniques, fall prevention strategies, and identification of fall-related risk factors.

各村按 1:1 的比例被随机分配到干预组（参与者定期接受下肢力量和平衡锻炼以及健康教育）或对照组（参与者接受常规护理）。随机分配是通过中央计算机化程序按县分层进行的。随机化过程将由独立于研究的人员进行，并在全部基线调查完成后开始。

Villages were randomly assigned in a 1:1 ratio to the intervention group, in which participants received regular lower limb strength and balance exercises and health education, or to the control group, in which the participants received usual care. Randomization was stratified according to county with the use of a central computerized process. Independent researchers will conduct the randomization process and begin after all baseline surveys are completed.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集将通过纸质问卷进行，数据采集完毕后将进行电子化录入。所有电子化数据包括电子化的调查问卷数据和实验室检测数据都将被储存于哈尔滨医科大学安全并有密码保护的服务器上；数据将长期保存于哈尔滨医科大学。所有纸质问卷以及知情同意书将被保存在哈尔滨医科大学带锁文件柜中。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection will be carried out through paper-based questionnaires, and will be entered electronically after data collection is completed. All electronic data, including electronic questionnaire data and laboratory test data, will be stored in a safe and password-protected server of Harbin Medical University; the data will be stored in Harbin Medical University for a long time. All paper questionnaires and informed consent forms will be kept in a locked filing cabinet at Harbin Medical University.
