研究目的:

通过利用BOLD-fMRI、rs-fMRI、DTI研究缺血性脑卒中单侧瘫痪患者大脑运动激活区的动态变化、运动功能网络连接的变化与脑白质纤维结构变化,对比分析远程居家康复治疗与传统康复治疗前后两种康复措施对缺血性脑卒中偏瘫患者大脑功能与结构重组的影响,探讨远程居家康复治疗对缺血性脑卒中偏瘫患者运动功能恢复的机制。  

Objectives of Study:

To compare the influences on brain motor function and restructuring of ischemic stroke patients with hemiplegia after telerehabilitation and traditional rehabilitation, and to explore motor functional recovery mechanism of telerehabilitation therapy among ischemic stroke patients with hemiplegia by learning changes of brain motor activation area, motor functional network connection and brain white matter fiber structure of ischemic stroke patients with hemiplegia based on BOLD-fMRI, rs-fMRI and DTI.

纳入标准:

Inclusion criteria

排除标准:

1. GCS评分<15,MMSE量表提示有痴呆,或精神障碍等疾患,不能配合检查及治疗的患者
2. 非缺血性脑卒中或既往缺血性脑卒中造成的运动或感觉功能障碍的患者
3. 合并有恶性肿瘤、严重心、肝、肾和造血系统等严重原发性疾病的患者
4. 既往有认知障碍、精神病史、药物滥用及酗酒史的患者
5. 前臂及小腿皮肤有感染或破溃的患者
6. 安置心脏起搏器、金属支架、钢板、关节等金属或易受电流脉冲影响的植入物的患者
7. 妊娠、哺乳期妇女或近期有生育计划的患者
8. 病前长期从事音乐演奏或键盘操作者
9. 有幽闭恐惧症者
10. 不能完成基本疗程,依从性可能不好者(即不能坚持治疗)及难以随访者

Exclusion criteria:

1. GSC score was under 15, confirmed dementia by MMSE assessment, with mental disorders and cannot cooperate with examination and treatment;
2. With neurological disability was not induced by cerebral infarction or with neurological disability induced by historical cerebral infarction;
3. Combined with malignant tumor, severe primary disease of heart, liver, kidney and hematological system;
4. With cognitive disorder, history of psychosis, substance abuse, history of alcoholism;
5. with skin infections and burst in antebrachium and crus;
6. With metal implant such as cardiac Pacemaker, metal stent, steel plate;
7. With gestation or lactation period or fertility plan;
8. Engaged in music playing or keyboard professionals for a long time before the stroke.
9. With claustrophobia.
10. Unable to complete the basic course of treatment, with poor treatment re adherence or unable to follow-up
