研究目的：

运用皮肤活检技术确定大连地区健康成人表皮周围神经密度及汗腺自主神经分布密度正常值及确定糖尿病周围神经病患者的表皮周围神经密度及汗腺自主神经分布密度诊断切点值。并应用多状态Markov模型，模拟糖尿病足的自然史，将糖尿病足的自然病程分为糖尿病周围神经病变超早期诊断，糖尿病足压力异常，糖尿病足溃疡形成3个阶段，分析糖尿病足各发展阶段的影响因素，计算糖尿病足各状态间的转移概率和转移强度，为开展糖尿病足分阶段防治及预测病情发展提供依据，促进糖尿病足更加早期有效预防和治疗，从根本上减少糖尿病足病变的发生。

Objectives of Study：

Skin biopsy technique was used to determine the normal values of peripheral epidermal nerve density and sweat gland autonomic nerve distribution density in healthy adults in Dalian area, and the diagnostic cut-off values of peripheral epidermal nerve density and sweat gland autonomic nerve distribution density in diabetic peripheral neuropathy patients. Moreover, multi-state Markov model was applied to simulate the natural history of diabetic foot, and the natural course of diabetic foot was divided into three stages: ultra-early diagnosis of diabetic peripheral neuropathy, abnormal pressure of diabetic foot, and formation of diabetic foot ulcer. The influencing factors of each stage of diabetic foot development were analyzed, and the probability and intensity of metastasis between each state of diabetic foot were calculated. To provide the basis for the stage prevention and treatment of diabetic foot and the prediction of disease development, promote the early and effective prevention and treatment of diabetic foot, and fundamentally reduce the occurrence of diabetic foot lesions.

Inclusion criteria

1. Determine the normal values of peripheral epidermal nerve density and sweat gland autonomic nerve distribution density in healthy adults in Dalian area. The inclusion criteria are: no symptoms of peripheral neuropathy, normal neurological examination, no history of drinking, normal blood routine, liver and kidney function, normal ions, no diabetes, Sjogren's syndrome, systemic lupus erythematosus, sarcoidosis, rheumatoid arthritis, celiac disease, vitamin B12/folic acid deficiency, thyroid disease, HIV, hepatitis B, hepatitis C infection in healthy adults.
2. To determine the diagnostic cut-off point value of peripheral epidermal nerve density and sweat gland autonomic nerve distribution density in patients with diabetes peripheral neuropathy. The inclusion criteria are:
1) Individuals with symptoms of peripheral nerve damage such as numbness, pain, and sensory abnormalities in their hands and feet;
2) Individuals with reduced or absent sensation of acupuncture, temperature, vibration, pressure, or ankle reflexes;
3) The examination of nerve conduction velocity shows abnormalities in the amplitude, latency, or conduction velocity of lower limb sensory/motor nerves. If one of the above criteria is met, it can be selected.

排除标准：

1.确定大连地区健康成人表皮周围神经密度及汗腺自主神经分布密度正常值。其排除标准为：有周围神经病变的症状，神经系统检查异常，有饮酒病史，血常规、肝肾功、离子异常，有糖尿病、干燥综合征、系统性红斑狼疮、结节病、类风湿关节炎、乳糜泻、维生素B12/叶酸缺乏、甲状腺疾病、HIV、乙肝、丙肝感染的成人。 2.确定糖尿病周围神经病患者的表皮周围神经密度及汗腺自主神经分布密度诊断切点值。其排除标准为： 1）年龄>80 岁； 2）心功能分级Ⅲ~Ⅳ级； 3）严重的肝肾功能不全者（内生肌酐清除率<50 ml/min，谷丙转氨酶或谷草转氨酶超过正常上限 3 倍以上）； 4）局部皮肤存在破损、溃疡； 5）利多卡因过敏史； 6）凝血功能异常； 7）下肢水肿； 8）已确诊为非糖尿病引起的周围神经系统疾病； 9）中枢神经系统疾病，如脑梗死、帕金森病、格林-巴利综合症等； 10）外周血管疾病； 11）结缔组织病，如干燥综合征、系统性红斑狼疮、白塞病、结节病、系统性血管炎； 12）不配合或有精神障碍者； 13）乳糜泻（麦胶性肠病）； 14）莱姆病。

Exclusion criteria：

1. Determine the normal values of peripheral epidermal nerve density and sweat gland autonomic nerve distribution density in healthy adults in Dalian area. The exclusion criteria are: adults with symptoms of peripheral neuropathy, abnormal neurological examination, history of drinking, abnormal blood routine, liver and kidney function, ions, diabetes, Sjogren's syndrome, systemic lupus erythematosus, sarcoidosis, rheumatoid arthritis, celiac disease, vitamin B12/folate deficiency, thyroid disease, HIV, hepatitis B, hepatitis C infection. 2. To determine the diagnostic cut-off point value of peripheral epidermal nerve density and sweat gland autonomic nerve distribution density in patients with diabetes peripheral neuropathy. The exclusion criteria are: 1) Age>80 years old; 2) Heart function grading III to IV; 3) Severe liver and kidney dysfunction (endogenous creatinine clearance rate<50 ml/min, alanine aminotransferase or aspartate aminotransferase exceeding the normal upper limit by more than three times); 4) Local skin damage and ulcers; 5) History of lidocaine allergy; 6) Abnormal coagulation function; 7) Lower limb edema; 8) It has been diagnosed as peripheral nervous system disease caused by non diabetes; 9) Central nervous system diseases, such as cerebral infarction, Parkinson's disease, Guillain Barre syndrome, etc; 10) Peripheral vascular disease; 11) Connective tissue diseases, such as Sjogren's syndrome, systemic lupus erythematosus, Behcet's disease, sarcoidosis, and systemic vasculitis; 12) Those who do not cooperate or have mental disorders; 13) Chlamydia (glial enteropathy); 14) Lyme disease.
