研究目的：

尝试构建不同单病种的整合型居家护理服务模式，形成居家护理服务的质量评鉴体系，探索居家护理服务的成本核算方式，此三者共同构成居家护理服务网络体系，形成一套行之有效的运行模式，从而改善慢病患者临床结局，减轻家庭负担，缓解医疗资源供需矛盾。最终为今后居家护理服务推广和相关政策制定提供依据，为机构间居家护理服务的质量评鉴提供工具，为政府合理制定服务价格，完善相关补偿政策提供参考。

Objectives of Study：

This study is to build different single diseases nursing service model, to establish nursing service quality evaluation system, and to explore the cost accounting of nursing service model, which helps to improve clinical outcomes, easing the burden on families and coordinating the contradiction between supply and demand of health resources. Finally the extended care will promote and provide a basis to carry out the policy of service, provide inter-agency extending nursing service quality evaluation tools, assist the government to establish reasonable prices, and improve the relevant compensation policy.

纳入标准：

Inclusion criteria

排除标准：

1）失语者；2）住院天数≤10天；3）同时患有其他严重的疾病；4）自理能力重度依赖；5）住院期间转至另一医疗单位者；6）同时参与其他相关研究项目者。

Exclusion criteria：

(1) aphasia;
(2) length of hospitalization<10 days;
(3) coexisting severe diseases (renal failure, heart failure, and end-stage diseases);
(4) premorbid dependence;
(5) transferred to another unit during hospitalization;
(6) involved in other research programs.

使用计算机生成的随机计划执行。分配给干预组/对照组的任务被密封在不透明的信封中。密封信封不透明，且在外观和大小和重量相等，给参与者的编号，使他们进入研究。只有在按照纳入标准筛选合格者后，信封才能打开，并提供知情同意。编写信封和实现随机分配的研究人员独立分配，不参与招募参与者、实施干预或测量结果。采用区组随机化分组和分配隐藏指定参与者确保每个参与者被分配到每个组的机会均等，最小化影响和偏见的研究人员或参与生产，并加强与干预方案的效果之间的关系。
