建立睡眠-觉醒障碍队列，在真实世界中研究睡眠-觉醒障碍的影响因素、转归、治疗疗效、与慢性病之间的关系，丰富睡眠-觉醒障碍的病因机制、提供防治诊治方案和临床决策指导。

Objectives of Study：

To establish a cohort of sleep-wake disorders with patients with sleep disorders such as unable to sleep, not waking up, and restless sleep, and to study the influencing factors, outcomes, treatment efficacy, and relationship with chronic diseases of sleep-wake disorders in the real world, so as to enrich the etiological mechanism of sleep-wake disorders and provide prevention and treatment plans and clinical decision-making guidance.

Inclusion criteria

排除标准：

1.无法完成自评问卷的患者； 2.拒绝本研究的患者； 3.研究者认为存在不合适参加本研究的情况。

Exclusion criteria：

1. Patients who could not complete the self-assessment questionnaire; 2. Patients who refused the study; 3. The investigators considered that there were circumstances that made participation in the study inappropriate.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：	我们设计了纸质CRF表，按照纸质的CRF建设好EDC系统，研究者负责采集每位受试者每次随访的信息至EDC系统中，数据管理人员进行数据管理。
