研究目的:

通过TCD/TCCD对脓毒性休克患者进行连续脑功能监测,明确脓毒性休克患者发生脑血管调节功能障碍的危险因素有哪些;观察脓毒性休克患者谵妄组与非谵妄组的脑血流动力学的动态改变差异与相互关系;随访分析脓毒性休克患者初始复苏治疗后脑血流动力学的动态改变差异与远期神经认知功能的关系。  

Objectives of Study:

Continuous brain function monitoring was performed on septic shock patients by TCD/TCCD to determine the risk factors of cerebral vascular regulation dysfunction in septic shock patients. To observe the difference and correlation of dynamic changes of cerebral hemodynamics between delirium group and non-delirium group in septic shock patients. The relationship between the dynamic changes of cerebral hemodynamics and long-term neurocognitive function in patients with septic shock after initial resuscitation was analyzed.

纳入标准:

Inclusion criteria

排除标准:

(1)年龄小于18周岁。(2)精神障碍病史或开颅手术史。(3)神经系统疾病(明确的颅内病变,如脑出血、蛛网膜下腔出血、颅脑创伤、中风、颅内感染等)。(4)肝衰竭合并肝性脑病可疑。(5)正在服用抗抑郁或抗癫痫药物。(6)药物或毒物中毒,酒精依赖。(7)妊娠。 (8)听力、视力严重受损的。(9)颈部血管检查、头颅 MRI/CT 及 TCD 检查发现异常者(如颈动脉斑块,血栓等)。(10)TCD/TCCD在颞窗无法探查血流信号者。文献支持来源(基于既往前瞻性观察研究为基础)

Exclusion criteria:

(1) Under the age of 18. (2) History of mental disorders or craniotomy. (3) neurological diseases (definite intracranial lesions, such as cerebral hemorrhage, subarachnoid hemorrhage, craniocerebral trauma, stroke, intracranial infection, etc.). (4) Hepatic failure combined with hepatic encephalopathy is suspicious. (5) Taking antidepressant or antiepileptic medications. (6) Drug or poison poisoning, alcohol dependence. (7) Pregnancy. (8) Severely impaired hearing and vision. (9) Cervical vascular examination, head MRI/CT and TCD examination found abnormalities (such as carotid artery plaque, thrombosis, etc.). (10) TCD/TCCD could not detect the blood flow signal in the temporal window. Sources of literature support (based on previous prospective observational studies)
