研究目的：

以2019年四川省中国糖尿病并发症研究青羊区调查的2型糖尿病患者中部分对象为研究人群（45岁-75岁，血糖、体重控制不佳者），探索给予膳食和身体活动的个性化建议，对血压等指标开展3个月数字化管理的模式；比较短期干预中给予膳食模式指导后血糖水平的变化，评估该短期干预对患者行为养成的影响，探索有利于血糖管理且可持续的川人膳食模式，从而预防或延缓并发症。  

Objectives of Study：

Taking some subjects of type 2 diabetes patients investigated in Qingyang District of China diabetes Complications Research in 2019 as the research population (45 to 75 years old, with poor control of blood sugar and weight), to explore the mode of giving personalized suggestions on diet and physical activity, and carrying out three-month digital management on blood pressure and other indicators. To explore the impact of providing dietary pattern guidance on blood sugar control in short-term interventions, as well as the impact of short-term interventions on patient behavior development and sustainability, to obtain a sustainable dietary pattern for Sichuan people that can control blood sugar and complications.

排除标准：

1.有体力活动禁忌症者； 2.既往有精神病史者； 3.不愿参加本次研究者。

Exclusion criteria：

1. Individuals with contraindications to physical activity; 2. Previous history of mental illness; 3. Unwilling to participate in this study.
