研究目的：

构建一个基于临床治疗状态的普适性新生儿疼痛评估量表，并进行临床推广应用，从而规范住院新生儿疼痛评估行为，为采取镇痛措施和评价镇痛效果提供依据。  

Objectives of Study：

This study aim to develop an universal neonatal pain assessment scale based on clinical state and popularize it in clinical application, so as to standardize the pain assessment behavior of hospitalized neonates and provide the basis for taking analgesic measures and evaluating the analgesic effect.

纳入标准：

Inclusion criteria

排除标准：

1.兰氏新生儿疼痛评估量表在急性操作性疼痛中的应用：
（1）疼痛评估时使用镇静镇痛药物的患儿；
（2）中枢神经系统严重损坏（III/IV脑室内出血）可能影响疼痛表现的患儿；
（3）濒临死亡的患儿；
（4）机械通气的患儿；
（5）肌张力低下的患儿；
（6）意识障碍的患儿。
2.兰氏新生儿疼痛评估量表在术后疼痛中的应用：
（1）新生儿面部畸形、肢体残疾；
（2）患有神经肌肉性疾病，如缺血缺氧性脑病
（3）术后行机械通气
（4）病情危重或处于病情突变期。
3.兰氏新生儿疼痛评估量表在机械通气疼痛中的应用：
（1）新生儿面部畸形、肢体残疾；
（2）意识障碍；
（3）患有神经肌肉性疾病；
（4）连续使用神经肌肉阻滞剂。
4.兰氏新生儿疼痛评估量表在持续疼痛中的应用：
（1）新生儿面部畸形、肢体残疾；
（2）患有神经肌肉性疾病；
（3）濒临死亡的患儿；
（4）意识障碍的患儿。

Exclusion criteria：

1. Application of Lan's Neonatal Pain Scale in Acute Procedural Pain:
(1) neonates who used sedatives and analgesics;
(2) neonates with severe damage to the central nervous system (III/IV intraventricular hemorrhage) that may affect pain presentation;
(3) dying neonates;
(4) neonates with mechanical ventilation;
(5) neonates with hypotonia;
(6) neonates with consciousness disorder
2.pplication of Lan's Neonatal Pain Scale in Postoperative Pain:
(1) neonates with facial deformity or physical disability;
(2) neonates with neuromuscular diseases, such as hypoxic ischemic encephalopathy
(3) neonates with mechanical ventilation
(4) critically ill neonates or in the stage of disease mutation.
3. Application of Lan's Neonatal Pain Scale in Mechanical Ventilation Pain:
(1) neonates with facial deformity or physical disability;
(2) neonates with disorder of consciousness;
(3) neonates with neuromuscular diseases;
(4) neonates who use neuromuscular blockers continuously.
4. Application of Lan's Neonatal Pain Scale in Chronic Pain:
(1) neonates with facial deformity or physical disability;
(2) neonates with disorder of consciousness;
(3) neonates with neuromuscular diseases;
(4) dying neonates.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者原始观察记录，将数据及时、完整、正确、清晰的载入报告表。录入采用相应的数据库系统双人双机录入，之后对数据库进行两边对比。电子数据文件分类保存，并有多个备份保存于不同的记录介质上，妥善保存，防止损坏。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The investigator shall load the data into the report form timely, completely, correctly and clearly according to the original observation records of the subjects.The corresponding database system was used for the input of two-person and two-machine, and then the two sides of the database were compared.Electronic data files are classified and stored with multiple backups on different recording media to prevent damage.
