研究目的：

探讨建立以社区人群为基础、以早期发现社区老年慢性病患者并进行规范化管理为目的，与国际接轨的、有中国特色、规范化、系统化的医院—社区一体化慢性病管理模式，为中国慢性病防治管理模式在全国的推广提供经验。我们的最终目标是改善和提高老年慢性病患者自我管理水平，从而预防或延缓疾病进展，提高生活质量，减轻社会经济负担。  

Objectives of Study：

Based on established on the basis of community people, the early detection of community in elderly patients with chronic diseases and standardized management for the purpose, in line with international standards, with Chinese characteristics, standardization, systematic hospital - community integrated model of chronic disease management, for the Chinese to provide experience for chronic disease prevention and control management model in the national promotion. Our ultimate goal is to improve the old patients with chronic disease self-management level, thereby preventing or delaying disease progression and improve the quality of life, society to reduce the economic burden.
