研究目的：

探究艾司氯胺酮对胸科手术患者整体炎症状态，血浆BMP4/NOG/促抗炎因子水平，外周单个核细胞内BMP4信号激活程度和促抗炎倾向，以及单核细胞的极化状态和功能表型的影响，并评估其临床应用的安全性，上述证据将为艾司氯胺酮在的抗炎机制提供临床依据。  

Objectives of Study：

To investigate the effect of esketamine on the systemic inflammation and the plasma expression pattern of BMP4/NOG/pro- and anti-inflammatory cytokines; Besides, the peripheral blood mononuclear cells (PBMC) was extracted to detect the activation of BMP4 signaling and the inflammation status; Moreover, the functional phenotype and polarized pattern of the monocytes were also evaluated. All evidence above will provide clinical evidence of the esketamine's anti-inflammatory property.

纳入标准：

Inclusion criteria

排除标准：

1.不同意参与本研究者； 2.对艾司氯胺酮过敏的患者； 3.有心脏大血管疾病史的患者 例如动脉粥样硬化 高血压病； 4.有神经系统疾病致颅内压升高； 5.未经治疗或者治疗不足的甲状腺功能亢进（甲亢）患者 ； 6.有或曾经有过严重的精神障碍病史的患者； 7.青光眼患者； 8.术前胸片提示肺部感染或合并有其他重要脏器疾病的患者； 9.既往有药物滥用史，尤其是吗啡类药物滥用史或长期使用糖皮质激素的患者； 10.术前检查有白细胞计数升高、中性粒细胞比值升高，或降钙素原升高，高于正常值上限的患者； 11.当患者手术术式改变或出血量大于 1000ml 或手术时间超过 6 小时的手术患者； 12.在入选研究前的 3 个月内参与了其他临床试验。

Exclusion criteria：

1. Patients who do not agree; 2. Patients who are allergic to esketamine; 3. Patients with a history of cardiac macrovascular disease such as atherosclerosis and hypertension; 4. Patients with neurological disease and increased intracranial pressure; 5. Patients with untreated or under-treated hyperthyroidism; 6. Patients who have or have had a history of severe mental disorders; 7. Patients with glaucoma; 8. Patients with preoperative chest radiographs suggesting pulmonary infection or other significant organ disease; 9. Patients with a history of drug abuse, especially morphine abuse or long-term glucocorticoid use; 10. Patients with increased white blood cell count, elevated neutrophil ratio, or elevated calcitoninogen above the upper limit of normal on preoperative examination; 11. Patients with a change in surgical procedure or bleeding volume greater than 1000 ml or surgical time longer than 6 hours; 12. Patients who participated in other clinical trials within 3 months prior to study enrollment.
