研究目的:

牙合架作为一种模拟人体咀嚼器官结构和功能的装置,自从1805年被发现后一致不断更新来满足更高的需求。其允许医生节约宝贵的椅旁时间,增加患者的舒适度,放大并充分照亮工作空间以及提高治疗效率。采用牙合架辅助设计制作的修复体在戴入口腔后可与机体达到更好的形态及功能上的协调。但传统上牙合架的过程,需要医生椅旁进行水平关系、垂直关系及铰链轴关系记录,精确性也受医生操作影响。而数字化牙合架的提出为临床医生简化了这种上牙合架的过程,复制了机械牙合架的功能,同时可以多角度观察动态上下颌相对运动,明显方便了口腔咬合调整,减少了修复体的椅旁适应时间。 然而,利用数字化虚拟牙合架进行颌位关系记录和转移的一大痛点是熟练应用虚拟牙合架获取患者稳定牙合位的技术敏感性高。因此,进一步探索正畸治疗和咬合修复过程中利用虚拟牙合架进行数字化颌位关系记录与转移的准确性成为了一大研究热点。 若能证明将3D面扫与口扫拟合转移虚拟牙合架方法的精确性,则可以帮助正畸医生通过一个更加简单而准确的方式将虚拟患者数字化,无需传统的面弓转移和咬合关系记录,即可实现虚拟牙合架参数设置,并可轻松与市场上的大多数虚拟牙合架系统集成,在功能和美学上对患者进行全方位模拟,指导正畸临床诊断、设计和治疗。另外,借助口内扫描、面部扫描、CBCT等技术在建立了一个具有美学和功能的全要素虚拟患者后,进行远程医疗的可行性大大增加,可以在很大程度上改善医疗资源分配不合理等问题,真正实现互联网医疗。  

Objectives of Study:

The mechanical articulator can mimic the anatomical structure of the human maxillofacial region, which not only can fix the upper and lower jaw models at rest to help doctors observe and diagnose problems that are difficult to find clinically; but also can simulate the jaw movement to a certain extent to study the dynamic interrelationship of teeth more intuitively. The traditional mounting procedures of the mechanical articulator requires human involvement of the physician in steps including recording the horizontal relationship, vertical relationship and hinge axis relationship of the patient, and applying the traditional facebow to transfer to the articulator, which is difficult to avoid the accuracy error and time-consuming in this process. Therefore, with the continuous development of digital technologies such as virtual reality technology and computer technology, virtual articulator modules with different functions have appeared in many dental design software, and scholars at home and abroad have begun to study virtual articulator systems, which have improved the efficiency of traditional dental treatment and provided more possibilities for doctors. This clinical study will review the application steps and procedures, advantages and disadvantages, and future development of the virtual articulator technology in order to help more dental clinicians, technicians and researchers to master the technology, improve the efficiency of clinical treatment, and promote the development of the digital virtual articulator field.

Inclusion criteria

排除标准:

1.年龄小于 18 岁或大于 30 岁的患者; 2.患有系统性疾病或恶性肿瘤的患者; 3.患有精神类疾病或无法正常表达主观意见的患者; 4.牙列缺损或缺失及(或)伴有严重的牙体牙周疾病患者; 5.有正颌或整形手术史的患者; 6.依从性差的患者; 7.未复诊和失访病例; 8.口腔卫生极差; 9.孕妇。

Exclusion criteria:

1. Patients younger than 18 years or older than 30 years; 2. Patients with systemic diseases or malignant tumors; 3. Patients with mental illness or who are unable to express their subjective opinions normally; 4. Patients with denture defect or absence and/or serious periodontal disease; 5. Patients with a history of orthognathic or plastic surgery; 6. Patients with poor compliance; 7. No follow-up and missing cases; 8. Poor oral hygiene; 9. Pregnant women.

The plaster model was mounted on an Artex semi-adjustable occlusor using a traditional face bow record. This installation is then digitized using a benchtop scanner, which is imported into the exoCAD virtual scaffold module to measure the relevant indicators.

Geomagic software was used to fit the patient's 3D face scan and mouth scan images to determine the jaw position relationship, and then the hinge axis and plane landmarks were determined on the soft tissue with the help of the 3D face scan, which was imported into the virtual occlusal frame module in exoCAD to measure the relevant indicators.
