研究目的：

分析治疗耐甲氧西林葡萄球菌等革兰阳性球菌感染万古霉素的治疗药物浓度与该药有效性和安全性的相关性，制定用于不同患者群体包括特殊群体如老年人、儿童、肾功能减退者等，不同MIC水平的耐甲氧西林葡萄球菌等革兰阳性球菌感染患者万古霉素治疗方案，为制定中国成人及儿童患者革兰阳性菌万古霉素目标治疗药物浓度提供数据支持。  

Objectives of Study：

To obtain the efficacy and safety of vancomycin against infections caused by gram-positive cocci such as MRSA in China, and to study the effectiveness and blood trough concentration security zone of vancomycin. To Establish PPK and PK/PD model to formulate treatment plan in gram-positive infection patients in different populations and different MIC levels.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1）入选前72小时内曾接受耐甲氧西林葡萄球菌等革兰阳性球菌治疗有效的药物，如去甲万古霉素、替考拉宁、利奈唑胺、利福平、夫西地酸、替加环素、达托霉素等全身抗菌治疗超过24小时者；
2）MSSA或耐甲氧西林葡萄球菌等革兰阳性菌为定植菌者；
3）未进行万古霉素血药浓度监测者；
4）万古霉素疗程＜5日者（除血透以及腹透者以外）；
5）妊娠或哺乳妇女；
6）耐甲氧西林葡萄球菌等革兰阳性球菌合并革兰阴性菌感染时，同时合用氨基糖苷类或多黏菌素类等肾毒性药物的患者；
7）缺乏感染的临床及实验室评价指标，不能进行疗效和安全性评价者；
8）近1月参加其他临床试验者。

Exclusion criteria：

1. Any patient conforms to the following items is not the object of investigation;
2. Patients effectively treated by anti-gram-positive agents within 72 hours before being selected, such as norvancomycin,teicoplanin, linezolid, and fusidic acid, compound SMZ/TMP,tigecycline and daptomycin;
3. Patients with colonized gram positive cocci or not infected;
4. Vancomycin treatment within 5 days (Except hemodialysis and peritoneal dialysis patients);
5. Pregnancy or breastfeeding women;
6. Patients combined with aminoglycosides;
7. Patients lacking of clinical infection and laboratory evaluation index and could not be evaluated the clinical efficacy and safety;
8. Participat in other clinical trials within 1 months.
