研究目的：

本研究以韧性理论为指导，构建脑卒中受试者自我管理“实时支持”照护方案，利用可穿戴技术让患者佩戴能够实时采集运动、睡眠、血压、心率、血氧等数据的智能手表，对刚出院的脑卒中患者进行实时追踪、监护的同时为患者提供不间断的医疗服务和自我管理支持。研究为提高患者的长期自我管理能力，提升生存质量，优化疾病转归提供新的思路。  

Objectives of Study：

Guided by the resilience theory, this study constructed "real-time support" care program for stroke subjects' self-management. Wearable technology will be used to enable patients to wear smartwatches capable of real-time collection of movement, sleep, blood pressure, heart rate, blood oxygen and other data, so as to provide patients with continuous medical services and self-management support. This study provides new ideas for improving patients' long-term self-management ability, improving quality of life and optimizing disease outcome.

纳入标准：

Inclusion criteria

排除标准：

1. 既往有精神病史或现存精神障碍；
2. 有严重伴随疾病，如严重肝肾功能不全、严重心律失常、频繁心绞痛、心力衰竭、心肌梗死等。

Exclusion criteria：

1. patients with a history of mental illness or existing mental disorders;
2. there are serious concomitant diseases, such as severe hepatic and renal insufficiency, severe arrhythmia, frequent angina, heart failure, myocardial infarction, etc.

随机方法（请说明由何人用什么方法产生随机序列）：

按照抽签分组的方法将每天预出院的患者按照1：1：1的比例随机分到智能手表干预组、运动手环组和对照组。随机化过程如下：按照事先决定的数字1为智能手表干预组，数字2为运动手环组，数字3为对照组，将写有数字1、数字2、数字3的纸条放入由对项目研究设计不知情的研究助理准备的密封、不透明的信封内，每位研究对象依次抽取信封内的纸条，根据抽中的数字决定该患者是智能手表干预组、运动手环组还是对照组。依此类推，直至每组纳入61名研究对象。

According to the predetermined number 1 as the smartwatch intervention group, number 2 as the exercise bracelet group, and number 3 as the control group, slips of paper with numbers 1, 2 and 3 are put into sealed and opaque envelopes prepared. Each subject choose the slips of paper in turn.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

资料收集员指导患者填写问卷并收集客观数据，问卷填写完成后当场收回，当场检查，如有不清楚、漏填、错填的地方，当场与研究对象核实、更正，客观数据存在疑问的地方反复核实，保证数据的完整性和真实性，对资料收集员设盲；数据录入阶段，一个唯一的识别号码将被分配给每个患者，两名研究助理使用Epidata V.3.1软件双人录入并再次检查，任何不清楚或缺失的信息将与原始文件和医疗记录进行交叉核对。对数据录入人员设盲。纸质问卷中的数据当天录入电脑中，数据录入完成后的纸质问卷将被封存在华西护理学院办公室资料保存柜中，数据将同时拷贝在加密的移动硬盘以及台式电脑中，防止数据丢失。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The data collector instructed the patients to fill in the questionnaire and collect objective data. After filling in the questionnaire, the data collector will check the data on the spot. If there are unclear, missing or wrong filling, the patients will correct them on the spot, and the objective data will be checked repeatedly to ensure the integrity and authenticity of the data. During the data entry phase, a unique identification number will be assigned to each patient, two research assistants will enter and double check using Epidata V.3.1 software, and any unclear or missing information will be cross-checked with the original documents and medical records. Blind data entry personnel. The data of paper questionnaire will be recorded into the computer in 24 hours. After data entry, the paper questionnaire will be sealed in the data storage cabinet in the office of West China School of Nursing in West China Hospital, and the data will be copied into the encrypted mobile hard disk and desktop computer at the same time to prevent data loss.
