研究目的：

术后认知功能障碍(POCD)是一种严重的术后并发症，介导其发生发展的分子机制仍不清楚。目前越来越多的研究提示炎症反应参与了POCD的发生，POCD易感个体术后炎症反应异常增强,而具有强大抑制炎症反应作用的糖皮质激素受体（glucocorticoid receptor，GR）是维持机体炎症反应在适度水平的关键。各种因素引起的GR基因表观遗传学差异影响GR信号正常发挥作用可能是POCD易感性个体差异的背后原因。本项目通过整体神经行为测试，将术后病人区分为POCD与非POCD人群，运用分子生物学方法从GR基因启动子甲基化和组蛋白乙酰化修饰，手术引起机体炎症反应水平到认知功能改变等多层次系统地阐明GR基因表观遗传学修饰与术后认知功能障碍易感性的关系。本项目的完成可从一个新角度为POCD发病机理提供更具有普遍意义的解释，同时为预防和治疗POCD提供新的策略。

Objectives of Study：

postoperative cognitive dysfunction (POCD) is a severe postoperative complication. The mechanism to the development of POCD is poorly understood. The increasing researches suggest that inflammation contributes to the development of POCD. Abnormal enhancement of inflammatory response was found in individuals vulnerable to POCD. Glucocorticoid receptor (GR), a powerful anti-inflammatory molecule, plays a key role in maintaining moderate inflammation levels. Epigenetic variance of GR caused by all kinds of factors affect the function of GR signal in the normal play, which maybe is the underlying reason of the susceptibility to POCD. This research will differentiate between POCD and no-POCD by neurobehavioral tests. Using the methods of molecular biology, we will try to elucidate the relationship of epigenetic modifications of GR and POCD at levels of GR gene methylation, histone acetylation, inflammation induced by surgery and cognitive function. The study may provide an universal explanation of pathogenic mechanism of POCD and offer new target and orientation for POCD prevention

Inclusion criteria

排除标准：

①简易精神状态量表(Minimum Mental State Examination，MMSE)评分<23分；②受教育年限<7年；③术前生化检查提示脑血管疾病、肾功能障碍或活动性肝病；④术前合并糖尿病；⑤术前有明确的神经系统或精神疾病史或服用相应药物史；⑥有酗酒史或药物依赖史；⑦长期使用糖皮质激素史；⑧体重指数>35；⑨服用镇痛或抗抑郁药物；⑩有严重的视力、听力障碍或因其他原因与访视者无法交流。

Exclusion criteria：

Mini-Mental State Examination (MMSE) score<23;; peptic ulcer disease, cardiac-cerebral vascular disease, diabetes mellitus; neurological or psychiatric disorders; history of allergic reactions to NSAIDs; history of drug and alcohol abuse; hepatic and/or kidney dysfunction; BMI>35; patients on analgesics or antidepressants; inability to comply with the study protocol or procedures.
