研究目的：

建立危重症患者临床研究队列，描述ICU后综合征的发生现况及相关风险因素，开发早期风险预警系统，优化建立以信息连续、措施规范、知识共享为特征的ICU重症患者过渡期护理模式。  

Objectives of Study：

To establish a cohort of critically ill patients, describe the occurrence of post-ICU syndrome and related risk factors, develop an early risk warning system, and optimize the establishment of a transitional care model for ICU patients characterized by continuous information, standardized measures, and knowledge sharing.

纳入标准：

Inclusion criteria

排除标准：

1. 入住ICU前有身体残疾，生活完全不能自理； 2. 有认知、听力或语言表达障碍，无法正确理解问卷内容； 3. 拒绝参加者。

Exclusion criteria：

1. Physically disabled before being admitted to the ICU, unable to take care of themselves at all; 2. Having cognitive, hearing or language barriers, unable to correctly understand the content of the questionnaire; 3. Rejection of participants.
