研究目的：

本研究以年龄≥65 岁的老年维持性血液透析患者为研究对象，利用磁共振采集胸部随呼吸运动的动态影像、制作动态电影，随后对磁共振电影图像分割得到动态肺部 Mask 图像，通过计算机深度学习建模，评估老年血液透析患者膈肌结构和功能指标，定量该人群膈肌功能状况。以心血管不良事件和死亡为终点事件构建风险预测模型并进行模型验证。通过体外实验初步探讨 HIF-1α是否通过BNIP3介导线粒体自噬，从而导致膈肌萎缩。该研究为评估老年维持性血液透析人群的膈肌功能提供具体参考，并初步探索其可能的分子机制，便于早期发现高危患者、早期干预，以降低其心血管不良事件的发生率和死亡率，改善患者生活质量，大大减少医疗支出。  

Objectives of Study：

In this study, elderly maintenance hemodialysis patients aged >= 65 years were screened as the research objects. Dynamic images of chest movement with breathing were collected by magnetic resonance images and dynamic movies were made. Then dynamic lung Mask images were obtained by segmentation of magnetic resonance images, and the structure and function indexes of diaphragm were evaluated by deep learning. The diaphragm function of the population was quantified. A risk prediction model with cardiovascular adverse events and death as endpoints was constructed and verified. In vitro experiments were conducted to investigate whether HIF-1α leads to diaphragmatic atrophy through mitochondrial autophagy mediated by BNIP3. This study provides specific reference for assessing diaphragm function in elderly maintenance hemodialysis patients and preliminarily explores its possible molecular mechanism, to facilitate early detection and early intervention of high-risk patients of diaphragm dysfunction,to reduce the incidence and mortality of cardiovascular adverse events, and to improve patients' quality of life and reduce medical expenditure.

纳入标准：

Inclusion criteria

排除标准：

1.过去3月内有机械通气病史； 2.过去3月内有急性心肌梗死病史； 3.FEV1＜70%预计值，且/或正在使用治疗肺部疾病的药物： 如β2受体激动剂、M受体阻滞剂、糖皮质激素、白三烯受体调节剂、抗组胺药； 4.肺纤维化； 5.免疫抑制治疗的患者； 6.明确存在膈肌麻痹的患者； 7.近5年恶性肿瘤病史； 8.肝功能衰竭：Child-Pugh B、C级； 9.不宜行胸部核磁共振检查的外源性植入物术后； 10.精神障碍、认知功能障碍等不能配合行量表评估者； 11.近3月参加其他临床研究者。

Exclusion criteria：

1. History of mechanical ventilation within 3 months; 2. A history of acute myocardial infarction within 3 months; 3. FEV1 < 70% of expected value, and/or are using lung disease drugs such as beta-2 agonists, M blockers, glucocorticoids, leukotriene receptor modulators, antihistamines; 4. Pulmonary fibrosis; 5. Patients on immunosuppressive therapy; 7. History of malignant tumor within 5 years; 8. Liver failure: Child-Pugh grade B and C; 9. After the operation of exogenous implants that are not suitable for chest MRI examination; 10. Mental disorders, cognitive dysfunction, etc., can not cooperate with the scale assessment; 11. Participated in other clinical researchers within 3 months.
