研究目的：

采用聚焦解决模式，结合国内外过渡期管理最新文献和相关疾病管理最新指南循证、入院即为出院准备的过渡期管理理念，构建慢性心力衰竭患者出院后居家自我管理实时支持方案；聚焦积极问题解决的慢性心衰患者过渡期健康状况及居家自我管理的实时监测、追踪、预警与支持；构建数字医疗背景下，慢性心衰患者过渡期管理的新模式。  

Objectives of Study：

Combining the latest literature on transitional management and the latest guidelines on disease management at home and abroad, and the concept of transitional management of hospital admission as preparation for hospital discharge, we constructed a wearable device based real-time self-management support program for chronic heart failure patients after hospital discharge, and conducted a randomized controlled trial based on this program; to monitor, track, warn and support the health status of chronic heart failure patients during the transitional period and their self-management at home in real time; to build a new model of transitional management for chronic heart failure patients in the context of digital healthcare.

1.痴呆患者； 2.精神障碍患者； 3.聋哑患者； 4.瘫痪患者。

Exclusion criteria：

1. Dementia patients; 2. Patients with mental disorders; 3. Deaf and mute patients; 4. Paralytic patients.
