研究目的：

研究盐酸戊乙奎醚预处理对减细胞术和腹腔热化疗后肾功能的影响。  

Objectives of Study：

To study the effect of penehyclidine hydrochloride preconditioning on renal function after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy.

药物成份或治疗方案详述：

选择择期行CRS联合HIPEC术的患者进行单中心随机、对照、双盲试验，根据是否应用PHC进行预处理，随机分为PHC组和对照组，PHC组于诱导前10分钟予以静注PHC（0.01mg/Kg稀释到3ml）行预处理，对照组于诱导前10分钟静注等容量的生理盐水，两组于术前、术毕、术后1-7天每天抽血进行肌酐（Cr），尿素氮（BUN）检测，计算肾小球滤过率（Cockroft-Gault方法），于术前、术毕、术后24h测定尿中性粒细胞明胶酶相关脂钙蛋白(NGAL)和肾损伤分子(KIM)-1。 

Description for medicine or protocol of treatment in detail：

Patients who were selected to receive CRS combined with HIPEC were selected for a single-center randomized, controlled, double-blind trial, and were randomly divided into PHC group and control group according to whether or not they were pretreated with PHC. The PHC group was pretreated with intravenous injection of PHC (0.01mg/Kg diluted to 3ml) 10 minutes before induction. The control group was injected with equal volume of normal saline 10 minutes before induction, and blood was drawn daily for creatinine (Cr) and urea nitrogen (BUN) detection before, after and 1-7 days after operation, and glomerular filtration rate was calculated (Cockroft-Gault method). Urinary neutrophil gelatinase-associated lipocalcin (NGAL) and renal damage molecule (KIM)-1 were measured before, after and 24h after surgery. 

纳入标准：

Inclusion criteria

排除标准：

1.术前存在肾功能不全（KDOQI分期≥4期，eGFR30mL/min/1.73m2）； 2.近1月内腹腔手术史； 3.青光眼； 4.自身免疫性疾病，近1月服用免疫抑制剂或糖皮质激素治疗； 5.近1周在服用抗氧化剂、乌司他丁等药物。

Exclusion criteria：

1. Preoperative renal insufficiency (KDOQI stage >= 4, eGFR < 30 mL/min/1.73m2); 2. History of abdominal surgery within the past 1 month; 3. Glaucoma; 4. Autoimmune disease with immunosuppressant or glucocorticoid therapy in the past 1 month; 5. Taking the antioxidant ulinastatin and other drugs for the past week.
