研究目的：

主要研究目的：评价胫骨横向骨搬运技术治疗糖尿病足溃疡是否能有效提高创面愈合率。 次要研究目的：探索影响胫骨横向骨搬运技术临床疗效的相关因素；补充完善胫骨横向骨搬运技术在治疗糖尿病足溃疡时候的手术适应症和禁忌症，相应可能出现的手术并发症。  

Objectives of Study：

OBJECTIVE: To evaluate whether the treatment of diabetic foot ulcer with tibial transverse bone manipulation can effectively improve the wound healing rate. Objective: To explore the factors affecting the clinical efficacy of tibial transverse bone transport technique; to supplement the surgical indications and contraindications of tibial transverse bone handling in the treatment of diabetic foot ulcer, and the corresponding possible surgical complications.

药物成份或治疗方案详述：

三、胫骨横向骨搬运治疗组方法：1．取仰卧位，常规铺巾消毒后，于胫骨中段前内侧弧形切开皮肤，切口长约12-15cm，逐层切开向下分离直至骨膜；2．沿胫骨内侧切开骨膜，向两侧完整剥离骨膜，并注意保护骨膜的完整性；3．在胫骨内侧面开骨窗，大小约12cm*2cm，注意保护周围组织，开骨窗过程中避免损伤及丢失髓腔骨髓；4．在骨窗内选取居中对称位置各拧入一枚3.0mm骨搬运外固定针，两针间距约5cm；5．用电钻在设定的骨窗界限钻取多孔，再用摆锯将钻孔连接成线分离搬运骨块（尽量不损伤骨髓）6.在骨窗近、远端胫骨侧钻孔并各拧入2枚4.0mm外固定针与骨针处于同一轴线上，安装并调整定制横向骨搬运外固定架于胫骨及搬移骨块上；6．冲洗伤口，逐层各层组织，包扎切口；手术完毕。

In the bone window near the distal tibial side of the drilling and the screw 2 4.0mm external fixation needle and bone needle in the same axis, install and adjust the custom lateral bone transport external fixator in the tibia and move the bone block; Rinse the wound, layer by layer organization, bandage incision; surgery is completed.

排除标准：

排除标准：①近期存在糖尿病酮症酸中毒、高渗性昏迷或严重慢性糖尿病并发症及其他应激情况如急性心、脑血管意外。 ②外周动脉病变中动脉以上无法再通者。③严重肝、肾功能损害者，如谷草转氨酶或谷丙转氨酶高于正常上限2.5倍； 肌酐高于正常上限1.5倍或血清肌酐浓度≥133 μmol/L(男 )/124 μmol/L(女 )或肌酐清除率< 60 mL/min。 ④严重的肺部感染、心肺功能不全控制不良者。 ⑤严重低蛋白血症者(白蛋白水平低于30 g/L)。⑥血红蛋白低于90 g/L者。

Exclusion criteria：

1. recent diabetic ketoacidosis, hyperosmolar coma or severe chronic diabetes complications and other stress conditions such as acute heart, cerebrovascular accident;
2. peripheral arterial lesions can not be more than the middle of the artery;
3. creatinine is higher than the normal upper limit of 1.5 times or serum creatinine concentration ≥133 umol/L (male)/124umol/L (female), such as aspartate aminotransferase or alanine aminotransferase, Or creatinine clearance <60 mL/min;
4. severe lung infection, cardiopulmonary dysfunction control poor;
5. severe hypoproteinemia (albumin levels below 30 g/L);
6. hemoglobin less than 90 g/L.
