研究目的:

第一部分:1.观察PSCIND患者在ST和DT条件下的脑激活和步态之间的关系,并探讨不同认知任务对脑活动、步态参数和认知功能的影响。 2.更详细地观察中风对脑血流的影响,从而进一步研究中风引起的认知功能障碍和双任务干扰的原理。 3.通过在双任务行走中设置不同的认知负荷,初步确认双任务训练的认知负荷范围,为双任务训练提供一个安全标准。 第二部分:1.应用 fNIRS和步态分析,通过研究头穴丛刺结合认知步态训练治疗的 PSCIND患者步态参数和颅内血红蛋白浓度变化,分析治疗前后步态变化和大脑皮层激活情况。 2.探索 PSCIND患者的功能连接变化,对针康法诱导脑卒中患者皮层出现正常可塑化改变的神经网络机制进行初步探究。  

Objectives of Study:

Part I: 1. To observe the relationship between brain activation and gait in PSCIND patients under ST and DT conditions and to explore the effects of different cognitive tasks on brain activity, gait parameters and cognitive function. 2. to observe in more detail the effects of stroke on cerebral blood flow, so as to further investigate the principles of stroke-induced cognitive dysfunction and dual-task interference. 3. initially confirm the cognitive load range of dual-task training by setting different cognitive loads in dual-task walking, so as to provide a safety standard for dual-task training. Part II: 1. Apply fNIRS and gait analysis to analyse gait changes and cortical activation before and after treatment by studying changes in gait parameters and intracranial haemoglobin concentration in PSCIND patients treated with cephalic acupoint plexus stabbing combined with cognitive gait training. 2. Explore the functional connectivity changes in PSCIND patients, and conduct preliminary investigation on the neural network mechanism of the normal plasticising changes in the cortex of stroke patients induced by the Acupuncture Kang method.

Inclusion criteria

排除标准:

1. 意识障碍,严重的视觉、听觉和语言障碍,以及其他健康评估无法完成。 2. 存在其他性质的脑部疾病(如多发性硬化症、脑瘤)和其他限制步态的疾病(如骨折、截肢)。 3. 存在其他原因导致的认知障碍,如阿尔茨海默病(AD)、路易体痴呆。 4. 伴随的疾病,如抑郁症、精神病性症状,以及存在乙醇、药物滥用或依赖的历史。 5. 不能遵守研究程序。 6. 失语症或其他影响发音的疾病。 7. 合并有肿瘤、严重器官、内分泌系统等疾病。

Exclusion criteria:

1. Impairment of consciousness, severe visual, hearing, and speech impairments, and inability to complete other health assessments. 2. The presence of brain diseases of other nature (e.g., multiple sclerosis, brain tumors) and other diseases that restrict gait (e.g., fractures, amputations). 3. Cognitive impairment from other causes, such as Alzheimer's disease (AD) and dementia with Lewy bodies. 4. Accompanying medical conditions, such as depression, psychotic symptoms, and a history of alcohol, drug abuse, or dependence. 5. Failure to follow research procedures. 6. Aphasia or other disorders that affect pronunciation. 7. Complicated with tumors, serious diseases of organs and endocrine system.

第一部分:通过python软件生成三个由1-14组成的数字表。参与者根据是否卒中患者、是否有认知功能障碍分为健康(对照)组、低认知组和高认知组,对于每一组,其中的参与者都将从一个独立的数字表中随机取得一个数字。每组的参与者将根据取得的随机数字再次分为两组,数字为奇数的一组则先执行简单双任务,数字为偶数的一组则先执行复杂双任务。 第二部分:通过python生成随机数字表,将84名参与者随机分配至三组。

Randomization Procedure (please state who generates the random number sequence and by what method):

Part I:Three number tables consisting of 1-14 will be generated by python software. Participants will be categorized into healthy (control), low-cognitive, and high-cognitive groups based on whether or not they are stroke patients, and whether or not they have cognitive dysfunction. For each of these groups, the participants in each of these groups will be randomly obtaining a random number from a separate number table. Participants in each group will be subdivided into two groups based on the random number obtained, with the group with an odd number performing the simple dual task first and the group with an even number performing the complex dual task first. Part 2: A table of random numbers was generated via python and 84 participants were randomly assigned to three groups.
