研究目的：

全国多中心医养整合照护服务试点项目计划对健康老龄化的服务体系进行前瞻性、可持续性的顶层设计，并且持续探索在基本维持现有资金支持水平的情况下，以创新方式探索相关工作模式，保障老年人的健康和照护需求及时得到满足。 针对目前医养结合亟待资源有效整合，而整合型服务需要数据支持的现状，配合各分中心所在地区医养结合居家养老服务试点等工作的开展，本试点项目将完成国际先进模式的本地化创新；实现老年人需求为导向，服务全人连续性管理；全面评估老年人健康干预的需求，评估整合照护资源的可及性，以老年人及其家属和服务团队的满意度、以及卫生经济学指标为评判标准；初步建立实施城市社区整合照护的方案流程。探索统一资费、打包服务（全人全年总额预付费）的可行性。以此推动不同地区健康养老服务业全面发展，将社区居家养老服务和高龄、衰弱、患有慢性疾病的老年人对医疗保健服务的刚需有效结合，最终建立成熟、规范、可复制、可持续的医养结合服务新模式。  

Objectives of Study：

The multi-center pilot project of ICOPE in China plans to carry out a prospective and sustainable top-level design of the service system for healthy aging, and continue to explore the relevant work modes in innovative ways to ensure that the health and care needs of the elderly are met in a timely manner, while maintaining the existing level of financial support. The pilot project aims to complete the localization innovation of the internationally advanced model; Realize demand-oriented service for the elderly and provide continuous management for the whole person; Comprehensively assess the needs of the elderly for health intervention, assess the accessibility of integrated care resources, and take the satisfaction of the elderly and their families and service teams, as well as health economic indicators as the evaluation criteria; The program process of implementing integrated urban community care was initially established. Explore the feasibility of unified pricing and package services (full year total prepayment). In order to promote the comprehensive development of the health care service industry in areas of diverse levels of development and resources, effectively combine the community home-based care service with the rigid needs of the elderly with old age, infirmity and chronic diseases for medical care services, and finally establish a mature, standardized, replicable and sustainable new model of integrated medical care services.

每个试点中心建立随机数字表（1-1500随机排序），由专人（只负责告知数字）掌管，符合入组条件的老年人，按照随机数字表的顺序获得随机数字。随机数字除以3，为整数、余1、余2，为对照组，整除的为干预组。

For each pilot site, establish a random number table (1-1500 random order), which is under the charge of a special person (who is only responsible for informing the number). The elderly who meet the enrollment conditions will obtain random numbers according to the order of the random number table. Divide the random number by 3 and those that can be divided without any remainder will be in the Intervention Group.
