研究目的：

1.评估两种治疗技术：浓缩纳米脂肪与脱细胞脂肪组织外基质在下肢慢性难愈性创面的疗效，并力争建立治疗慢性难愈性创面的新技术。 2.从临床治疗的角度阐明：纳米脂肪这种基质细胞混合物局部移植过程中天然的基质成分对干细胞发挥再生性修复作用的影响，为干细胞负载外基质作为干细胞局部移植的重要方法诱导组织原位组织再生提供可靠的临床证据。

Objectives of Study：

1. To evaluate the efficacy of concentrated nano-fat and decellularized adipose tissue extracellular matrix in the treatment of hard-to-heal wounds of lower extremities, and to establish a new technique for the treatment of chronic refractory wounds. 2. From the perspective of clinical treatment to clarify the effect of natural matrix components on the regenerative function of stem cells during local transplantation of nano-fat, a mixture of stromal cells, and provides reliable clinical evidence for the induction of in situ tissue regeneration by stem cells transplanting loaded with extracellular matrix.

纳入标准：

Inclusion criteria

排除标准：

1.心功能不全导致双下肢水肿者；
2.下肢静脉血栓的患者；
3.糖尿病病人血糖控制不佳者；指空腹血糖>=8mmol/L；或餐后2h血糖>=10.0mmol/L；或糖化血红蛋白（HbA1c）范围>= 9%；
4.最近三个月内参与过类似临床试验；
5.下肢动脉闭塞，需要接受下肢血管手术患者；
6.创面感染，需要抗生素全身治疗患者；
7.干性痂皮或坏死组织覆盖创面大于50%，且不适合或拒绝接受清创治疗患者；
8.免疫缺陷患者；
9.有创面恶变可能或伴有恶性病变的患者（慢性创面1年以上要做活检排除）；
10.创面面积大于100cm2患者；
11.深达骨质、关节腔、体腔的创面；
12.伴有窦道的创面；
13.严重脏器功能障碍，严重营养不良患者；
14.有精神或其他问题且无法自我管理设备的患者；
15.治疗顺应性差的患者。
16.研究者认为因其他原因不宜参加本临床试验者。

Exclusion criteria：

1. Lower limb edema caused by cardiac insufficiency;

2. Patients with lower extremity venous thrombosis ;

3. diabetic patients with poor glycemic control; Fasting blood glucose >=8mmol/L; Or 2h postprandial blood glucose >=10.0mmol/L; Or glycosylated hemoglobin (HbA1c) range >= 9%;

4. Have participated in similar clinical trials within the past three months;

5. Patients with lower extremity arterial occlusion requiring lower extremity vascular surgery;

6. Patients with wound infection requiring systemic antibiotic treatment;

7. Patients with dry crust or necrotic tissue covering more than 50% of the wound surface and unsuitable for or refusing to receive debridement treatment;

8. Immunocompromised patients;

9. Patients with the possibility of wound malignant transformation or with malignant lesions (chronic wounds that has lasted more than 1 year should be excluded by biopsy );

10. Patients with wound area greater than 100 cm2;

11. Deep bone, joint cavity, body cavity wound;

12. Wounds with sinus tract;

13. Patients with severe organ dysfunction and severe malnutrition;

14. Patients with psychiatric or other problems who are unable to self-manage the device;

15. Treat patients with poor compliance.

16. Those who were deemed by the investigator to be unfit to participate in the trial for other reasons.

由不参与随机的研究者采用SAS统计软件分层区组随机化的方法，按分中心进行分层，以1:1比例随机分为浓缩纳米脂肪试验组和脱细胞脂肪外基质试验组组。

The researchers who did not participate in the randomization used SAS statistical software with stratified block randomization method, stratified by sub-center, and randomly divided intent-to-treat population into concentrated nano fat treated group and decellularized adipose-derived matrix treated group at a ratio of 1:1.
