研究目的：

髋关节置换术的麻醉常复合腰丛阻滞，但腰丛阻滞因交感神经被阻滞，易致低血压，对术后认知功能潜在不良影响。竖脊肌平面阻滞（ESPB）是新近的一种阻滞技术，有研究ESPB的镇痛效果与腰丛相似。本研究将ESPB联合喉罩全麻用于老年髋关节置换术，并对麻醉效果、血流动力学等进行分析，评价超声引导下腰段ESPB的效果及对术后镇痛的影响。  

Objectives of Study：

Anesthesia for hip replacement is often combined with lumbar plexus block, but lumbar plexus block is prone to hypotension due to the blockage of sympathetic nerves, which has a potential adverse effect on postoperative cognitive function. Erector spinae plane block (ESPB) is a recent block technique, and studies have shown that the analgesic effect of ESPB is similar to that of the lumbar plexus. In this study, ESPB combined with laryngeal mask general anesthesia was used for elderly hip replacement, and the anesthesia effect and hemodynamics were analyzed to evaluate the effect of ultrasound-guided lower lumbar ESPB and its impact on postoperative analgesia.

纳入标准：

Inclusion criteria

排除标准：

1. 对研究中的任一药物存在使用禁忌、过敏者；
2. 下肢神经、肌肉病变者；
3. 对本研究过程难以配合完成操作者；
4. 穿刺部位感染或超声下无法辨别解剖结构或无法明确体表定位点；
5.凝血功能障碍者；
6. 转移癌引起的髋部骨折者；
7. 多发、多处创伤者；
8. 注射区感染者；
9. 存在重度认知障碍者；
10. 有阿尔茨海默病、中风和精神疾病等病史；
11. 严重的视力或听力障碍。

Exclusion criteria：

1. Those who have contraindications or allergies to any drug in the study;
2. Patients with neurological and muscle diseases of the lower extremities;
3. Those who are difficult to cooperate with the completion of the research process;
4. The puncture site is infected or the anatomical structure cannot be identified under ultrasound or the body surface positioning point cannot be clearly defined;
5. Those with coagulation dysfunction;
6. Hip fractures caused by metastatic cancer;
7. Patients with multiple and multiple traumas;
8. Infected persons in the injection area;
9. Those with severe cognitive impairment;
10. Have a history of Alzheimer's disease, stroke and mental illness;
11. Severe vision or hearing impairment.
