研究目的：

1.调查轻度失能老年人社会隔离现状、影响因素及认知体验； 2.基于社会生态系统理论构建轻度失能老年人社会隔离干预方案； 3.评价干预方案对降低轻度失能老年人社会隔离风险的效果。  

Objectives of Study：

1. To investigate the status quo, influencing factors and cognitive experience of social isolation in the elderly with mild disability; 2. To construct social isolation intervention program for mildly disabled elderly people based on social ecosystem theory; 3. To evaluate the effect of intervention program on reducing the risk of social isolation in the elderly with mild disability.

纳入标准：

Inclusion criteria

排除标准：

1.处于重症或疾病终末期； 2.因突发状况不能参加者； 3.因多种因素主动要求退出者。

Exclusion criteria：

1. In severe or end-stage illness; 2. Unable to participate due to unexpected circumstances; 3. Those who voluntarily request to withdraw due to various factors.
