研究目的：

本研究拟对需要机械通气的危重患者给予益生菌辅助治疗，分析其肺部及肠道微生物变化，以观察益生菌对肺部菌群多样性的影响情况，并确定这种多样性的改变是否与疾病的严重程度和死亡率相关。

Objectives of Study：

In this study, we plan to give the critical patients who need mechanical ventilation probiotics adjuvant treatment, analyze their lung and intestinal microbial changes, to observe the impact of probiotics on the diversity of pulmonary flora, and determine whether this diversity change is related to the severity of disease and mortality.

药物成份或治疗方案详述：

纳入标准：

Inclusion criteria

排除标准：

(1)机械通气时间＜5天；(2)免疫抑制（HIV，慢性免疫抑制药物，先前的器官或血液移植，绝对中性白细胞计数<500细胞/μl）；(3)有增加血源性感染的因素(风湿性心脏病，先天性心脏病，机械瓣膜，心内膜炎，血管内移植物或永久性血管内装置如血液透析导管，起搏器或除颤器)的病史；(4)不能或禁忌肠内营养。

Exclusion criteria：

1. Mechanical ventilation time < 5 days; 2. Immunosuppression (HIV, chronic immunosuppressive drugs, previous organ or blood transplantation, absolute neutrophil count<500 cells/ μl); 3. Have a history of factors that increase blood borne infection (rheumatic heart disease, congenital heart disease, mechanical valve, endocarditis, intravascular graft or permanent intravascular devices such as hemodialysis catheter, pacemaker or defibrillator); 4. Enteral nutrition cannot be or is contraindicated.

应用简单随机化分组方法，由专人将患者按照就诊顺序编号，从随机数字表第6行第1列开始，依次读取2位数作为一个随机数记录于编号下，然后将全部随机数从小到大排序，按照序号依次交替分为对照组与益生菌组。分配顺序对患者及临床医生隐藏。

The simple randomized grouping method was used. The patients were numbered by specially assigned person according to the order of visit. Starting from the sixth row and the first column of the random number table, two digits were read as a random number and recorded under the number.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：	所有基线数据和原始数据在获得后将立即、完整、准确、清晰地记录在CRF上。两名人员交叉检查以确保数据的准确性和清洁度。为了提高受试者的依从性，能够按照方案完成整个过程的受试者减少其他治疗费用。如参加者选择退出，会被要求提供理由并记录下来。本研究使用Microsoft Excel管理数据，所有研究数据将在每月最后一天录入该电子数据库。本数据库数据将直接输入IBM SPSS 25.0，便于统计分析。采用双人录入方式进行数据的输入和校对。同意书和纸质的数据收集表格将被妥善存储。
Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：	All baseline data and original data will be recorded on CRF immediately, completely, accurately and clearly after being obtained. Two personnel cross checked to ensure the accuracy and cleanliness of the data. In order to improve the compliance of the subjects, the subjects who can complete the whole process according to the scheme will reduce other treatment costs. If participants choose to withdraw, they will be asked to provide reasons and record them. This study uses Microsoft Excel to manage data, and all research data will be entered into the electronic database on the last day of each month. The data in this database will be directly input into IBM SPSS 25.0 for statistical analysis. Double entry mode is adopted for data input and proofreading. The consent form and the paper data collection form will be properly stored.
