研究目的:

探究不同吸入氧浓度下,驱动压导向的通气策略与传统肺保护性通气策略相比,对体外循环心脏手术患者肺功能的影响,为体外循环心脏手术术中合适吸入氧浓度及肺保护性通气策略的选择提供依据。  

Objectives of Study:

To explore the effects of driven pressure-oriented ventilation strategy compared with traditional lung protective ventilation strategy on lung function in patients undergoing cardiopulmonary bypass surgery under different inhalation oxygen concentrations, and to provide evidence for the selection of appropriate inhalation oxygen concentration and lung protective ventilation strategy during cardiopulmonary bypass surgery.

纳入标准:

Inclusion criteria

排除标准:

1.既往行胸肺部手术; 2.术前机械通气; 3.术前存在严重呼吸系统疾病者(如哮喘、慢性阻塞性肺病、急性呼吸衰竭等); 4.术前严重肝、肾、凝血功能异常(ALT>100U/L,AST>100U/L,血清胆红素>40μmol/L,血清白蛋白<30g/L, Cr>353μmol/L,BUN>14.3mmol/L,PT延长>6秒,INR>2.3); 5.颅内高压、气胸患者; 6.行微创手术者(胸腔镜辅助等)。

Exclusion criteria:

1. Previous thoracic and pulmonary operations; 2. Preoperative mechanical ventilation; 3. Patients with severe respiratory diseases before surgery (such as asthma, chronic obstructive pulmonary disease, acute respiratory failure, etc.); 4. Severe abnormalities of liver, kidney and coagulation before surgery (ALT > 100U/L, AST > 100U/L, serum bilirubin > 40μmol/L, serum albumin < 30g/L, Cr > 353μmol/L, BUN > 14.3mmol/L, PT > 6 s, INR > 2.3); 5. Intracranial hypertension and pneumothorax; 6. Minimally invasive surgery (thoracoscopic assistance, etc.).
