研究目的:

上肢运动功能障碍是缺血性脑卒中最主要的后遗症,严重影响患者的日常生活活动能力。临床上针对卒中后上肢运动功能障碍的治疗方法有作业治疗和强制性运动训练等治疗,但这些疗法临床效果有限,特别是对慢性期患者。目前国内外已经有相关研究将重复外周磁刺激和迷走神经刺激技术应用于脑卒中上肢运动功能障碍的治疗,但关于迷走神经磁刺激治疗脑卒中后上肢运动功能的研究鲜有报道,其治疗效果和机制尚不明确。本研究将通过收集迷走神经磁刺激治疗前后患侧上肢肌力、肌张力、上肢运动功能、wolf运动功能测试、日常生活能力观察治疗疗效;收集患侧上肢电生理结果(运动诱发电位潜伏期、波幅、脑血流量改变)和脑部神经递质功率的改变,探讨迷走神经磁刺激治疗脑卒中后上肢运动功能障碍的机制。本研究成果将能更好为脑卒中后上肢运动功能障碍的评估和治疗提供新思路,可补充对目前临床上脑卒中后上肢运动功能障碍评估和治疗的不足。  

Objectives of Study:

Upper limb motor dysfunction is the main sequelae of ischemic stroke, which seriously affects patients' ability of daily living activities. Clinical treatments for upper limb motor dysfunction after stroke include occupational therapy and mandatory exercise training, but the clinical effects of these therapies are limited, especially for chronic patients. At present, there have been relevant studies at home and abroad applying repetitive peripheral magnetic stimulation and vagus nerve stimulation technology to the treatment of upper limb motor dysfunction after stroke. However, few studies have reported on the treatment of upper limb motor function after stroke by vagus nerve magnetic stimulation, and its therapeutic effect and mechanism remain unclear. In this study, the therapeutic effect was observed by collecting the muscle strength, muscle tone, upper limb motor function, wolf motor function test and daily living ability of the affected side before and after vagus magnetic stimulation treatment. The electrophysiological results (changes in motor evoked potential latency, amplitude, and cerebral blood flow) of the affected upper limb and the changes in neurotransmitter power of the brain were collected to explore the mechanism of vagus magnetic stimulation in the treatment of upper limb motor dysfunction after stroke. The results of this study will better provide new ideas for the evaluation and treatment of upper limb motor dysfunction after stroke, and can supplement the current clinical evaluation and treatment of upper limb motor dysfunction after stroke.

Inclusion criteria

排除标准:

①生命体征不稳定;②颅内恶性肿瘤或未破裂的动脉瘤;③癫痫发作史或脑积水;④怀
孕;⑤颅内金属植入或安装心脏起搏器;⑥合并严重心血管及肝肾功能疾病者;⑦其余外周磁刺激
治疗禁忌证。

Exclusion criteria:

① Unstable vital signs; ② Intracranial malignancy or unruptured aneurysm; ③ History of seizures or hydrocephalus; (4) Pregnancy; (5) Intracranial metal implantation or installation of cardiac pacemaker; ⑥ Patients with severe cardiovascular, liver and kidney diseases; ⑦ Other contraindications of peripheral magnetic stimulation.
