研究目的：

该项目主要研究说话瓣膜联合呼吸康复训练在重型气管切开患者的临床疗效，两种方法联合治疗，能否有效提高患者的疗效，改善患者的吞咽功能、言语功能、提高生活质量，缩短带管时间，患者自我形象能否得到改变，是否值得临床推广应用。  

Objectives of Study：

The main focus of this project is to investigate the clinical efficacy of speech valve combined with respiratory rehabilitation training in patients with severe tracheotomy. The combination of the two methods can effectively improve the patient's efficacy, enhance swallowing and speech functions, improve quality of life, shorten catheterization time, change the patient's self-image, and whether it is worthy of clinical promotion and application.

纳入标准：

Inclusion criteria

排除标准：

入ICU前已经气切；严重认知功能障碍；气道大量分泌物；气胸未治疗；肺部存在中重度不（或不完全）可逆性病变；颈椎不稳或颈段脊髓损伤；BMI＞30；上呼吸道梗阻；全喉切除术；食管反流。

Exclusion criteria：

Aircut was performed before admission to the ICU; Severe cognitive impairment; Massive secretion in the airway; Untreated pneumothorax; Moderate to severe pulmonary insufficiency (or incompleteness) reversible lesions; Cervical instability or cervical spinal cord injury; BMI>30; Upper respiratory tract obstruction; Total laryngectomy; Esophageal reflux.
