研究目的：

1. 主要目的：探究围手术期静脉使用低剂量或重复剂量（术前10mg/术前10mg+术后10mg）地塞米松方案对非糖尿病患者TJA术后空腹血糖水平，餐后2小时血糖水平的影响； 2. 次要目的：探究围手术期静脉使用低剂量或重复剂量（术前10mg/术前10mg+术后10mg）地塞米松方案对非糖尿病患者TJA伤口并发症、切口感染、假体周围感染、90天再入院率、90天死亡率及地塞米松相关不良反应的影响。  

Objectives of Study：

1. Primary purpose: To evaluate the effect of low or repeated dose of dexamethasone regimens (10 mg before operation/10 mg before operation and 10 mg after operation) used intravenously during perioperative period on fasting blood glucose level and blood glucose level 2 hours after meal after TJA in patients without diabetes; 2. Secondary purpose: To evaluate the effect of low or repeated dose of dexamethasone regimens (10 mg before operation/10 mg before operation and 10 mg after operation) used intravenously during perioperative period of TJA on wound complications, incision infection, periprosthetic infection, 90 day readmission rate, 90 day mortality and dexamethasone related adverse reactions in patients without diabetes.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1.糖尿病患者或患其他导致血糖升高疾病的患者；
2.术前因各种原因长期使用激素（包括地米，氢化可的松，甲强龙等等）/术前6个月内使用过类固醇或免疫抑制药物；
3.对糖皮质激素有过敏史/本研究中使用药物既往有不良反应的患者；
4.严重的肝肾功能不全的患者；
5.炎性疾病，包括感染性关节炎，类风湿性关节炎, 强直性脊柱炎, 系统性红斑狼疮等；
6.糖皮质激素使用禁忌症的患者，如活动性的溃疡，慢性感染，重度骨质疏松，精神疾病，低钾血症等；
7.关节翻修，关节感染的患者；
8.对本实验使用的其他药物过敏的患者。

Exclusion criteria：

1. Patients with diabetes or other diseases that lead to elevated blood glucose;
2. Steroid or immunosuppressive drugs used within 6 months before operation;
3. Patients with a history of allergy to glucocorticoids or with adverse reactions to the drugs used in this study;
4. Patients with severe liver and kidney dysfunction;
5. Patients wtith inflammatory diseases, including infectious arthritis, rheumatoid arthritis, ankylosing spondylitis, systemic lupus erythematosus, etc.;
6. Patients with contraindications to glucocorticoid use, such as active ulcer, chronic infection, severe osteoporosis, mental disease, hypokalemia, etc.;
7. Patients with joint revision and joint infection;
8. Patients who are allergic to other drugs used in this experiment.
