研究目的：

细针抽吸洗脱液测定甲状腺球蛋白(thyroglobulin in fine-needle aspirate fluid，FNA-Tg)对于甲状腺乳头状癌侧颈区淋巴结转移有较高的诊断价值，且与术后病理诊断符合率高，技术简单易掌握。本研究通过超声引导下FNA-Tg结合FNAB在术前评估甲状腺乳头状癌侧颈淋巴结是否存在，目的在于： 1. 采用该新技术能够对可疑侧颈淋巴结进行评估； 2. 统计分析临床试验数据，探讨FNA-Tg的诊断阈值（cutoff值）； 3. 标准化新技术操作流程。  

Objectives of Study：

The determination of thyroglobulin in fine-needle aspirate fluid (FNA-Tg) has high diagnostic value for lymph node metastasis in the cervical region of papillary thyroid carcinoma, and the coincidence rate with postoperative pathological diagnosis is high, and the technique is simple and easy to master. In this study, FNA-Tg combined with FNAB under ultrasound guidance was used to evaluate the existence of cervical lymph nodes on the side of thyroid papillary carcinoma before surgery. The purpose was to: 1. The new technique can be used to evaluate the suspicious lateral neck lymph node; 2. Statistical analysis of clinical trial data, to explore the diagnostic threshold of FNA-Tg (cutoff value); 3. Standardize the operation process of new technology.

纳入标准：

Inclusion criteria

排除标准：

1. 颈部超声检查未见甲状腺可疑结节及可疑颈部淋巴结； 2. 未进行淋巴结FNA-Tg检测； 3. 术后病理结果显示PTC合并滤泡癌、鳞状细胞癌或低分化癌者； 4. 合并有淋巴结结核、淋巴结原发疾病或其他部位肿瘤转移至淋巴结者； 5. 有其他部位肿瘤病史者； 6. 凝血功能不正常、患者不能配合等。

Exclusion criteria：

1. No suspicious thyroid nodules or suspicious cervical lymph nodes were found in cervical ultrasound examination. 2. FNA-Tg was not detected in lymph nodes. 3. Postoperative pathological results showed that PTC complicated with follicular carcinoma, squamous cell carcinoma or poorly differentiated carcinoma; 4. Patients with lymph node tuberculosis, primary lymph node disease or metastasis to other sites of tumors; 5. Patients with a history of tumors in other sites; 6. Abnormal coagulation function, patients can not cooperate, etc.;

甲状腺乳头状癌伴侧颈淋巴结肿大的患者，且超声提示具有以下特征之一的可疑淋巴结：淋巴结形态呈圆形、类圆形等不规则形状；回声不均匀，伴有局部性的回声增高或降低；囊性病变或者钙化；淋巴结门结构缺失；融合的淋巴结；周围血流信号增多。

In patients with papillary thyroid carcinoma with lateral neck lymph node enlargement, the ultrasonography indicated suspicious lymph nodes with one of the following characteristics: irregular shape of lymph nodes, such as round or quasi-round; The echo is not uniform, with local increase or decrease of the echo。

容易混淆的疾病人群（即与目标疾病不易区分的一种或多种不同疾病，应避免采用正常人群对照的病例-对照设计）：

甲状腺良性结节或甲状腺术后病理显示PTC;合并滤泡癌、鳞状细胞癌或低分化癌者；可疑合并有淋巴结结核、淋巴结原发疾病或其他部位肿瘤转移至淋巴结者。

Benign thyroid nodules or postoperative thyroid pathologic findings of PTC with follicular carcinoma, squamous cell carcinoma or poorly differentiated carcinoma; Suspected lymph node tuberculosis, lymph node primary disease or other sites of tumor metastasis to the lymph node.
