研究目的：

1.评估慢性病患者生活质量、健康素养，明确慢性病控制率及健康管理需求； 2.构建人口老龄化背景下智慧赋能的老年健康服务平台； 3.应用智慧赋能的老年健康服务体系并评价，为推广实施提供基于实验证据的政策建议。  

Objectives of Study：

1. To assess the quality of life and health literacy of patients with chronic diseases, and clarify the chronic disease control rate and health management needs; 2. To develop a service platform for the elderly with wisdom and empowerment under the background of population aging; 3. To apply and evaluate the wisdom empowered elderly health service system, and provide policy suggestions based on experimental evidence for promotion and implementation.

排除标准：

1.患者有严重精神认知疾病； 2.有严重并发症者。

Exclusion criteria：

Patients with documented cognitive impairments or severe medical complications.
