研究目的：

观察和评价依托泊苷胶囊联合甲磺酸阿帕替尼片治疗复发和转移小细胞肺癌患者的有效性和安全性

Objectives of Study：

Observation and evaluation the recurrence and metastasis small cell lung cancer treatment s efficacy and safety with etoposide plus apatinib

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

Inclusion criteria

1. Pathological or cytological diagnosed small cell lung cancer;
2. Aged 18-75, man or women;
3. According to standard RESCIST1.1, evaluation target lesion, conventional CT≥ 2cm , the spiral CT≥ 1cm. Random two weeks lesions measurement;
4. Had a systemic treatment of recurrence or metastasis in patients with small cell lung cancer (patients had palindromia and metastasis during 6 month with systemic chemotherapy;patients had palindromia and metastasis during 6 month with systemic chemotherapy, appear again after treatment with the original plan, palindromia and metastasis);
5. Expected survival time ≥3 month;
6. ECOG 0-2;
7. Baseline laboratory tests meet the following criteria: Bone marrow function ANC ≥2.0×10^9/L; PLT ≥100×10^9/L; HB≥95g/L;  Liver function: STB, CB≤ ULN×1.5; ALT and AST≤ ULN×2.5 (In the absence of liver metastasis) or ≤ ULN×5(had a liver metastasis); Renal function: Cr≤ULN; Ccr ≥ 60 ml/min; Blood coagulation function: (PT INR)≤ ULN×1.5;
8. Women of childbearing age are willing to using appropriate methods of contraception during the test and 8 weeks after having the last experimental drug, and should have a pregnancy check before test if necessary, the result should be negative; For men, must be agree to use appropriate methods of contraception or have sterilization surgery during the experiments or at the end of the experiments after 8 weeks;
9. The patients were willing to join in this study, and sign the informed consent, good adherence, cooperate with the follow-up.

排除标准：

1. 需要治疗活动期的脑转移（定义为<4周的稳定和/或具有症状和/或需要抗惊厥剂或甾体类和/或软脑膜疾病的治疗）；
2. 存在较严重的肿瘤侵犯、压迫所致主支气管、叶支气管狭窄或堵塞、上腔静脉综合症，或中等量以上的恶性胸、腹腔积液或心包积液，尚未控制者；
3. 严重心血管疾病，包括内科治疗无法控制的高血压、不稳定性心绞痛、过去6月内有心肌梗死病史、充血性心力衰竭>纽约心脏协会心脏疾病分类（NYHA）3级以及严重的心律失常；
4. 入组前4周内出现≥CTCAE （见附件6）3级的出血；
5. 需要长期应用抗凝剂或维生素K拮抗剂，如华法林、肝素或其类似物，但不包括以预防为目的使用小剂量华法林（≤1mg /d）或阿司匹林（≤100mg/d）除外；
6. 严重感染，需要静脉使用抗生素、抗真菌或抗病毒治疗；
7. 妊娠、可能妊娠患者、未实行有效避孕措施且有妊娠意愿的患者以及哺乳期患者；
8. 已知存在的遗传性或获得性出血及血栓倾向（如血友病人，凝血机能障碍，血小板减少，脾功能亢进等）；
9.长期未治愈的伤口或骨折；
10.入组前4周内接受过重大外科手术或出现重度创伤性损伤、骨折或溃疡；
11.具有明显影响口服药物吸收的因素，如无法吞咽、慢性腹泻和肠梗阻等；
12.尿常规提示尿蛋白≥ ++，或证实24小时尿蛋白量≥1.0 g；
13.具有精神类药物滥用史且无法戒除者或有精神障碍的；
14. 研究者判断其他可能影响临床研究进行及研究结果判定的情况；

Exclusion criteria：

1. Need cure brain active metastasis (defined as < 4 weeks of stability and/or have symptoms and/or need anticonvulsants agent or nonsteroidal and/or the treatment of pia mater disease);
2. Exist serious tumor invasion, Pressure caused by the main bronchus, bronchial stenosis or jam,superior vena cava syndrome, or the amount of the above malignant thoracic and abdominal cavity effusion and pericardial effusion, has not yet been controller;
3. Serious cardiovascular disease, including medical treatment cannot control high blood pressure, unstable angina, over the past six months has a history of myocardial infarction, congestive heart failure, heart disease classification > New York heart association (NYHA) level 3 and severe arrhythmia;
4. Occurred ≥CTCAE ( annex 6) level 3 bleeding within 4 weeks before the group;
5. Need long-term daily use of anticoagulants or vitamin K antagonists, such as warfarin and heparin or its analogues, but not for the purpose of prevention using small dose of warfarin (≤1 mg/d ) or aspirin (≤100 mg/d) except;
6. Severe infection, need intravenous antibiotics, antifungal and antiviral treatment;
7. pregnancy, possible pregnancy patients, had not effective birth control and the will of the patients with pregnancy and lactation patients;
8. Known inherited or acquired bleeding and thrombosis tendency (such as hemophilia, blood coagulation dysfunction, thrombocytopenia, hypersplenism, etc.);
9. Long-term did not cure wound or fracture;
10. Acceptd major surgery or severe traumatic injury, fractures, or ulcer before into the group received within 4 weeks;
11. Have a significant factors that affect oral drug absorption, such as unable to swallow, chronic diarrhea and intestinal obstruction, etc;
12. Urine routine prompt urine protein ≥+ +; or confirm 24 hours urinary protein quantity≥ 1.0 g;
13. Has a history of psychiatric drugs abuse and can't quit or have a mental disorder;
14. Researchers determine other possibility that affect the clinical research and research results.
