研究目的:

评估上颌骨复杂缺损患者应用计算机辅助设计多段式腓动脉嵌合穿支皮瓣修复缺损的效果及安全性。  

Objectives of Study:

To evaluate the effect and safety of reconstruction of maxillary complex defects with multi-segment fibula chimeric peroneal artery perforator free flap assisted by digital techniques.

药物成份或治疗方案详述:

采用韦伯-弗格森切口,在导板的引导下完成肿瘤切除。在截骨导板引导下切取腓骨游离皮瓣,携带皮岛一起取下并分割成5段腓骨段。按照就位导板放置各个腓骨段,以恢复面中部美学轮廓,恢复牙槽高度与牙弓弧度以及修复眶下缘的缺损。腓骨第一段为近心段修复颧骨及眶外侧骨壁,第二、三、四段修复上颌骨至对侧牙槽峭,第五段终末段反折修复眶下缘及内眦。为了能使节段5反折向上,达到最佳的修复角度,在4、5节段间去除一部分附着腓骨形成血管蒂。多段腓骨瓣的应用使得颧骨、上颌骨体及牙槽嵴的修复更加符合生理三维轮廓,而第五段腓骨瓣的反折得以修复眶下缘缺损。个性化钛网重建眶底。腓骨瓣所携带的皮岛用于闭合口腔。腓总动脉及伴行静脉与颞浅动静脉吻合。 

Description for medicine or protocol of treatment in detail:

Weber-Ferguson incision was used to expose the maxilla, orbital floor, and zygomatic bone. Then the maxillary cutting guiding plate was placed to guide the tumor ablation and maxillary resection. A segmented fibula flap was harvested with a peroneal artery perforator free flap under the guidance of the fibular cutting guiding plate. The proximal and distal ends of the remaining fibula must be long enough (>5cm) to maintain the joint's stability. With guidance, the fibula flap was divided into six segments. Each fibula segment was placed according to the in-place guiding plate to restore the three-dimensional aesthetic contour of the middle face, the alveolar height, and the arch radian. Meanwhile, the peroneal artery perforator free flap was used to repair the oral cavity's defects. The titanium mesh was placed and fixed on the 6th segment to reconstruct the orbital floor. The peroneal artery and accompanying vein were anastomosed to the temporal artery and vein.

排除标准:

1.腓骨的骨密度不符合手术要求,如骨质疏松、骨外伤; 2.口内软组织缺损面积超过腓骨瓣可携带的最大皮瓣; 3.患者有其他手术禁忌症,如下肢外周动脉疾病的严重体征和症状,以及其他影响血管健康的局部或全身因素。

Exclusion criteria:

1. The bone density of the fibula does not meet the operational requirements, such as osteoporosis, bone trauma; 2. The area of intraoral soft tissue defects exceeds the largest skin paddle that can be carried by fibula flap; 3. The patient has other surgical contraindications, such as severe signs and symptoms of peripheral artery disease of the lower extremities and other local or systemic factors that affect vascular health.
